STATEMENT OF WORK
Replace Three Casement Windows.
Location Building: 1728 By: Patrick LeBorgne
WORK ORDER NUMBER: 67510
DATED: 11 March 2019
1. GENERAL INFORMATION:
This project is intended to replace three casement windows in accordance with all applicable National, Air Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, shop drawings, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, Unified Facilities Criteria (UFC) state and federal codes, base standards and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply. Effective immediately: Due to safety concerns the use of manhole fixed ladders on Hanscom AFB is prohibited for the purpose of entry and egress is prohibited. An alternative means is authorized (i.e. appropriate portable ladder/personnel lowering device).
2. SCOPE: Replace three casement windows.
• Remove and properly dispose of interior trim surrounding the three existing casement windows.
• Remove and properly dispose of EIFS (stucco siding) surrounding the three existing casement windows. Care should be taken to minimize the amount of EIFS (stucco siding) which is removed.
• Remove and properly dispose of the three existing casement windows. Each casement window measures approximately 66” wide and 70” high.
• Properly prepare the rough openings (per manufacturer’s instructions) for the installation of the three new casement windows ensuring that new casement windows are sealed to the building and weather tight
• Install three new white vinyl casement windows (per manufacturer’s instructions) ensuring that they are installed level, plumb, square, and operate properly. The three new white vinyl casement windows shall be Andersen 400 series Casement, Left-Right-Right, White- White/Pine White-Painted, with insulated laminated glass, Contemporary Folding White Hardware, and Insect Screen (Full Screen Aluminum White) or approved equals. New casement windows shall be fully functional with all necessary hardware.
• Install flashing (per manufacturer’s instructions) ensuring that the three new casement windows are leak proof and weather tight.
• Make all necessary repairs to EIFS (stucco siding) ensuring that new EIFS (stucco siding) matches the existing finish and color.
• Make all necessary repairs to interior drywall and interior wall covering (sound proofing panels).
• Install new extension jambs as required.
• Prime new extension jambs. Primer shall be a latex primer.
• Paint new extension jambs (2 coats). Paint shall be latex and shall match the existing color and finish.
• Install new interior trim. The new interior trim shall match the existing interior trim.
• Prime new interior trim. Primer shall be a latex primer.
• Paint new interior trim (2 coats). Paint shall be latex and shall match the existing color and finish.
3. GENERAL REQUIREMENTS:
Work includes, but is not limited to the following:
a. Provide resolutions of any technical problems that arise during the construction.
b. Should any change in scope be required, it must be coordinated with the Project POC and approved as a change order prior to performing the change in work.
c. Provide updates to Project POC. Any construction issues, changes to schedule, or schedule slippages with justification must be reported at the periodic updates.
d. The Contractor is solely responsible for compliance with the Occupational Safety and
Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor employees. In performing work under this contract on a Government installation, the contractor shall report any accidents involving Air Force property/equipment damage, or contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the contracting officer within five days.
e. Any modifications and addition on a facility that contains Lightning Protections System (LPS) This includes paint projects, roofing projects, projects requiring roof penetrations, installation of new HVAC or other metallic equipment, antenna installation, or other work.
Additions and modifications to the facility envelope require re-evaluation of the LPS for the completed facility as a whole. The LPS required by the addition or modification shall not be a simple addition to the LPS. The LPS must be considered a single entity and must comply with requirements of a single LPS upon completion. The LPS shall be inspected by a commercial, third-party inspector whose sole work is lightning protection, and shall be certified by this third-party inspector as compliant with AFI 32-1065 and NFPA 780, in that priority order. Projects calling for an LPS addition to an existing LPS project shall consider the configuration of the final LPS in the initial design. Projects of this type shall ensure the final LPS as a whole is compliant with AFI 32-1065 and NFPA 780, in that priority order. The same third-party inspector requirements shall apply.
f. Complete the “builder data inventory and assessment sheet” for newly installed assets, ensuring the bar code number is attached to the new equipment. Transfer bar codes from any removed equipment to the builder assessment ½ sheet form
g. Provide three (3) copies of O & M manuals and warranty letter
h. Period of Performance 90 days after award of contract

Attached please find results for the assessment performed on the date referenced above.
Unless otherwise noted in the attached report, the assessment performed met the requirements of Absolute Resource Associates Standard Operating Procedures or industry guidelines and standards for the investigation of Asbestos Containing Materials (ACM) and lead-based paint within a building. The Standard Operating Procedures for sampling and investigations are based upon OSHA and EPA standards and adhere to all state regulations. The results in this report pertain only to the samples as indicated on the chain of custody.
Absolute Resource Associates does not engage in “Lead Risk Assessments”, rather lead determinations within a facility. Absolute Resource Associates does not carry licenses for the Lead Risk Assessor.

II. Scope of Work The assessment was conducted on Feb. 28, 2019 at the request of Roland Hartz of Hanscom AFB. The object of the assessment was to sample for asbestos-containing material in B1728 and lead paint in B1521.
In B1728, the object of the assessment was to sample for asbestos prior to renovation of the windows in Room I (classroom). Materials were sampled that were likely to be impacted by the removal of the windows.
In B1521, the white painted interior support columns near the south entry were sampled for lead-based paint using an XRF.
Jason Muchmore of Absolute Resource Associates conducted the inspection.
The building assessment was conducted to establish the following:
● Assess the potential for Asbestos Containing Materials (ACM) in the building.
● Physically inspect building materials, sample and analyze materials.
● Collect environmental samples and analyze via Polarized Light Microscopy to determine if the material has ACM.
● Collect paint chip samples and analyze for lead paint using an X-Ray
Fluorescence (XRF) analyzer. Identify levels of lead-Paint (Pb) that exceed 1.00 mg/cm².
● Throughout the process, consult other qualified professionals if necessary or desired, III. Asbestos Findings: Building 1728, Room I Asbestos containing material (ACM) is material containing >1% asbestos.
The condition of ACM is categorized as Good, Fair, or Poor. The type of ACM is categorized as Surfacing (Surf.), Miscellaneous (Misc.), and Thermal System Insulation (TSI). The ACM is also determined to be either friable or non-friable.
The materials that were sampled that were likely to be impacted by the renovation of the windows included drywall, the sound proof panels on the side walls, and exterior stucco. There was no accessible joint compound observed in the immediate area surrounding the windows.

Samples were analyzed onsite using a portable XRF. The XRF was calibrated onsite prior to use.

VI. Limits of Liability The IAQ assessment does not cover concealed areas or items not inspected. The extent of the limited area also depends on the building construction and conditions, weather, building usage and other factors. Areas that were inaccessible during the investigation were not inspected. Due to the nature of the investigation and the limited data available, Absolute Resource Associates cannot warrant against undiscovered environmental liabilities.
Any use which a third party makes of this report, or reliance on decisions made based upon it, is the responsibility of such third parties. Absolute Resource Associates accepts no responsibility for damages, if any, suffered by any third party as a result of decisions made or actions based on this report.
The conclusions presented in this report represent the best technical judgment of Absolute Resource Associates based on the data collected from the work. The conclusions are based on the site conditions encountered by Absolute Resource Associates at the time the assessment was performed. The information and analysis contained in this report have been generated using the EPA /600/R-93/116 Method for the Determination of Asbestos in Bulk Building Materials. Materials or products that contain more than 1% of any kind or combination of asbestos are considered an asbestos containing building material as determined by the EPA. This Polarized Light Microscope (PLM) technique may be performed either by visual estimation or point counting. Point counting provides a determination of the area percentage of asbestos in a sample. If the asbestos is estimated to be less than 10% by visual estimation of friable material, the determination may be repeated using the point counting technique. The results of the point counting supersede visual PLM results. Results in this report only relate to the items tested. This report may not be used by the customer to claim product endorsement by NVLAP or any other U.S. Government Agency.
Laboratory results represent the analysis of samples as submitted by the customer. Information regarding sample location, description, area, volume, etc., was provided by the customer. Asbestos Identification Laboratory is not responsible for sample collection activities or analytical method limitations. Unless notified in writing to return samples, Asbestos Identification Laboratory discards customer samples after 30 days. Samples containing subsamples or layers will be analyzed separately when applicable. Reports are kept at Asbestos Identification Laboratory for three years. This report shall not be reproduced, except in full, without the written consent of Asbestos Identification Laboratory.
