STATEMENT OF WORK
FOR
Removal of in-place Hospital Morgue & Install of New Hospital Morgue
PREPARED BY
96 MDG Medical Equipment Management Office Eglin AFB, Florida 32542
As of 21 May 21
1. DESCRIPTION OF SERVICES. The 96th Medical Group (96 MDG) requires the installation of a morgue refrigerating unit. The contractor shall provide all management, materials, tools, equipment, and labor necessary to dispose of the current unit residing in the 96 MDG, as well as coordinate, install and test the new morgue refrigerating unit as outlined in this Performance Work Statement (PWS).
2. GENERAL REQUIREMENTS. The Contractor is responsible for removing and installing a new morgue-refrigerating unit.
2.1. The contractor is responsible to keep a neat and clean work area. All trash and debris must be disposed of properly at the end of each day.
2.2. Any damages done to the Government facility as a result of the contractor’s employees shall be repaired appropriately at no additional cost to the Government.
3. REMOVAL AND DISPOSAL REQUIREMENTS. The contractor shall remove the existing morgue refrigerating unit (Manufacturer: Thermo Electron Corporation Model: LMR-2-SO Serial Number: SO0804002) and provide a written statement confirming that all safety related preparations have been made, to include but not limited to the safe removal of Freon from the unit along with sanitization.
3.1. The contractor will be responsible for disconnecting electrical during the removal of the existing morgue and all electrical connections during the installation of the new unit.
3.2. Palletize the removed morgue refrigeration unit in preparation for Defense Reutilization and Marketing Office (DRMO) turn-in.
3.3. Provide all written documentation confirming that this unit has been rendered fully safe for both personnel and the environment
3.4. The Medical Logistics Flight will also be responsible for creating, retaining and filing the Defense Reutilization and Marketing Office turn-in document.
4. INSTALLATION REQUIREMENTS. The contractor is responsible for relocating new morgue currently palletized in Medical Logistics Warehouse to the install location. Logistics Warehouse and Morgue room are in the same facility on the ground floor.
4.1. The contractor shall uncrate and inspect the equipment to confirm all parts required were shipped and in good working order. Instruction manual located in the pallet containing the new morgue.
4.2. Equipment Staging: Equipment to be assembled and installed (MOPEC KI200 Refrigerator, 2-body Morgue, Side-Opening).
4.3. No Hospital personnel need to be present during the removal of the old nor the installation of the new morgue.
4.4. Contractor is responsible for all mechanical, plumbing, electrical, and HVAC trades throughout the removal and install. All work shall be inspected and approved by Hospital Facility Management and/or Medical Maintenance personnel
4.5. Contractor is responsible for all drywall finish work.
5. Additional Install Details
5.1. Contractor shall perform all finish drywall work to ensure no gaps between drywall and newly installed morgue.
5.2. Contractor is responsible for all electrical work for removal and install of morgue.
6. TESTING AND DOCUMENTATION REQUIREMENTS. The contractor shall test the installed equipment and provide documentation in accordance with the following:
6.1. Provide documentation to Hospital Facility Management and/or Medical Maintenance personnel of successful testing.
6.2. The installation of the new morgue unit will be considered successful only after the 96th Medical Group Facility Management and Medical Maintenance teams have provided their approval.
6.2.1. This approval will be in the form of a Medical Maintenance Equipment Inspection decal being affixed to the exterior of the new morgue refrigeration unit.
6.3. SUPPORT. The contractor shall provide status updates either in person or over telecom during each phase of the project to Hospital Facility Management and/or Medical Maintenance personnel.
7. TRAVEL. The contractor may determine, at their discretion, whether a visit to the facility is necessary prior to the installation to evaluate site readiness.
8. GENERAL INFORMATION.
8.1. Hours of Operation. The contractor shall perform the services required under this contract during the following hours: Monday thru Friday, 0700-1600, with the exception of U.S. Federal holidays (listed below). Performance may be required 7 days per week, including weekends and Federal holidays. The contractor will be notified seven calendar days in advance, if work is to be conducted outside the normal work hours. Additionally, if the contractor needs to work outside of the normal work hours, a request must be submitted to the Contracting Officer 7 calendar days before requested time.

8.2. The Force Protection Condition (FPCON) level may change for real-world events or for exercise scenarios that may prevent contractor employees from entering or leaving Eglin AFB until the situation has lessened. Depending upon the situation, contractor employees shall react to FPCON requirements and comply based on 96th Test Wing (96 TW) guidance provided through the Contracting Officer.
8.3. Continuation of Essential DOD Contractor Services. This service is determined to be non-essential for performance during crisis according to DODI 3020.41, Operational Contract Support, and paragraph 2.c. In the event of crisis, the contractor will be notified by the Contracting Officer of the need to discontinue services due to contingency, base closure or other causes. If notification to discontinue services is not received, the contractor will be expected to continue performance in accordance with this PWS.
9. GOVERNMENT UTILITIES. The Government will not provide any equipment, personnel, utilities or facilities to the contractor for the execution of this contract with the exception of a location to perform the duties at government facilities, meetings, training on the database and access to the infrastructure required for the successful install of the new morgue unit.
