SAFETY REQUIREMENTS

INDEFINITE DELIVERY/INDEFINITE QUANTITY (IDIQ)

BUILDING DEMOLITION & GENERAL DEMOLITION

1. This is a safety requirements package the contractor must comply with while working on

Eglin Air Force Base. The contractor must develop and submit a site-specific Safety and Health

Plan that pertains to the scope of the contracted work. The contractor’s Safety and Health Plan shall address in detail the items listed in below. If task specific procedures are incorporated into a company operating procedure/document, those procedures must be attached to the submitted

Safety and Health Plan for review. Only referencing the company's operating procedure/ document in the Safety and Health Plan is not adequate. If the company's operating procedure/ document is not attached; the submitted Safety Plan will be returned NOT ACCEPTED.

2. Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. The government shall assume no liability or responsibility for the contractor's compliance or noncompliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.

3. OSHA Inspection: You are susceptible to an OSHA inspection while working on base. If an

OSHA inspector visits your work site, notify the 96 TW Safety Office immediately at (850) 882-

2540 ext 4.

4. Mishap Notification for Contractors - AFI 91-202/AFMC SUP 1, CHAP 12: The contractor must have written mishap notification procedures that includes the following procedures: The contractor shall notify the 96 TW Safety Office telephonically at (850)-882-

2540 extension 4 during duty hours and (850)-883-4020 after hours, within one (1) hour after initial notification of all mishaps or incidents for damage to DoD property (material plus labor) and employee injuries as a result of the contract operation. A follow-up (written or e-mail) of this mishap/incident notification shall be sent within three calendar days to the Government

Representative, the Contracting Officer (CO) who will forward report to the 96 TW Safety

Office. The information that is not available at the time of the initial notification shall be provided within 20 calendar days after the mishap. Mishap notification shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Impound Authority Name and Phone Number (as applicable)

g. Nomenclature of equipment and personnel involved in accident/incident

h. Corrective actions (taken or proposed)

i. Other pertinent information

If requested by CO, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the investigating safety office. If the government investigates the mishap, the contractor and the subcontractors shall cooperate fully and assist the government personnel until the investigation is finalized and closed out.

5. Motor Vehicles and Traffic Safety – AFI 91-207: The contractor shall comply with all traffic regulations while operating a vehicle on the installation. Ensure safe operating condition of all contractor-owned vehicles. Unsafe and unserviceable vehicles shall be removed from service immediately. Seatbelt use is mandatory during vehicle operations. It is prohibited to use a cell phone while operating a motor vehicle except when using a hands-free device such as

Bluetooth. Texting while driving is prohibited. Personnel engaged in vehicle operations on the flightline shall be trained and certified in flightline procedures IAW AACI 13-203. The contractor is responsible for scheduling this training through the Base Operations Office (phone number: (850) 882-2614) with adequate leadtime so as not to interfere with contract schedules.

Certification shall be on AF Form 483 and must be in the possession of the vehicle operator when operating on the flightline or industrial area. The contractor shall ensure that all subcontractors comply with these requirements.

6. Protective Barriers/Warning Signs: All necessary precautions shall be taken to prevent injury to the public or damage to property of others. Barricades are to be provided by the contractor. Barricades must be erected before the work begins. If the barricades are in a roadway or walkway, blinking lights must be used after dark. Warning lights shall be maintained from dusk to sunrise around excavations, barricades or obstructions in areas. When the work is complete, the barricades must be removed from the job site. Personnel working in traffic environments will wear appropriate reflective clothing.

7. Housekeeping: 29 CFR 1926.25 and 1926.34: It is the responsibility of contractor personnel to maintain good housekeeping. The work site shall be kept in a neat and orderly condition. Loose lumber shall be piled in an orderly fashion with nails removed or bent over.

Scrap material shall be removed by the contractor at the end of each workday.

8. Confined Spaces: 29 CFR 1910.146: Permit-required confined space entry operations must be performed in accordance with 29 CFR 1910.146 and Air Force Occupational Safety and

Health Standard 91-25. Contact 96 TW/SEG (Elizabeth Martin) at (850) 882-7350 or 882-2540 extension 4 to coordinate confined space entry operations and contractor’s OSHA plan prior to commencement of work, no exceptions!

9. Excavation and Trenching, 29 CFR 1926.650-652: An Air Force Form 103 (Base Civil

Engineering Work Clearance Requests) shall be completed prior to any digging operation on

Eglin. This applies to any depression in the ground formed by earth removal. Work to be done on Eglin ranges contact the 96 TW Range Safety Office 882-4000. Excavation and trenching shall be in accordance with 29 CFR 1926.650. Adequate protection from a cave-in must be provided by shoring, sloping, planking, or a trench box. Employees shall not enter an unprotected trench. Additionally, excavated or other materials must be at least 2 feet back from the edge of a trench and a safe means of egress shall be provided within 25 feet of workers in a trench.

10. Fall Protection, 29 CFR 1926.500 – 503 and Scaffolding, 29 CFR 1926.450-454:

Contractor’s Safety and Health Plan shall address how they comply with the fall protection and scaffold standards, training and qualifications of personnel. Walking and working surfaces with unprotected sides or edges will be guarded or the employee otherwise protected from falling by way of a personal fall arrest system (PFAS) lifeline, lanyard, component strength and anchorage point). Scaffolds and support equipment shall meet the design and construction specifications of

29 CFR 1926.451.

11. Aerial Lifts: 29 CFR 1926.453: Aerial lifts shall be designed and constructed in conformance with applicable requirements of the American National Standards Institute (ANSI)

Standards. Ensure only trained and authorized person operate aerial lifts. Maintain and operate elevating work platforms in accordance with the manufacturer's instructions.

12. Cranes, Derricks, Hoists: 29 CFR 1926.1400 – 1501: Cranes are to be operated only by qualified and trained personnel. Operators shall follow manufacturer instructions while operating cranes. A designated competent person must inspect the crane and all crane controls before use.

13. Electrical Safety: 29 CFR 1926.400 – 442 and AFI 91-203: All temporary and permanent electrical work, installation, and wire capacities shall conform to the current National Electrical

Code in addition to all applicable federal, state, and local codes. Electrical wiring and equipment shall be a type listed by UL or other recognized listing agent. All temporary electrical wiring shall be adequately installed and placed so as to avoid physical damage from other operations.

Only qualified and certified electricians will perform electrical work. Electrical powered equipment will be properly grounded at all times. All 120-volt, single-phase, 15- and 20- ampere receptacle outlets on construction sites, which are not part of the permanent wiring of the building or structure and are in use by employees, shall have approved ground-fault circuit interrupters (GFCIs).

14. Control of Hazardous Energy - Lockout/Tagout (LOTO) Program: 29 CFR 1910.147 The contractor’s LOTO procedures must comply with 29 CFR 1910.147. Authorized personnel shall ensure that all hazardous energy (active or stored) is controlled when servicing and maintaining equipment or machines. Power shall be disconnected and all energy isolating devices will be locked out and/or tagged out before starting work. The authorized person will verify the isolation and the de-energizing of the equipment.

15. Demolition: 29 CFR 1926.850 – 860: Demolition activities shall be performed in accordance with ANSI Standard A10.6 (Safety Requirements for Demolition) and 29 CFR

1926.850. Removal of materials through floor openings shall comply with 29 CFR 1926.853.

16. Hazard Communication: 29 CFR 1910.1200: The contractor must comply with OSHA’s

Hazard Communication Program, which includes training of personnel. Contractors must provide Material Safety Data Sheets (MSDSs) for each chemical used at least 5 working days prior to start date. This includes, but is not limited to, all solvents, paints, adhesives, sealants, coatings, primers, mastics, etc. MSDSs must be the most current available.

17. Inclement Weather Plan: (US Army Corps of Engineer Safety and Health EM 385-1-1)

Lightning occurs frequently in the Eglin AFB area; Florida has the highest number of lightning strikes and casualties in the country. The contractor is required to have an inclement weather notification plan to ensure the protection of all contractor employees. Plan must meet requirements of US Army Corps of Engineer Safety and Health Manual, EM 385-1-1, and AFI

91-203, chap 11.

18. Sub-contractors: (AFI 91-202/AFMC Sup 1, Chap 12): The prime contractor performing work on Eglin Air Force Base shall include a clause in all contracts requiring the subcontractors to comply with the safety provisions of this contract. All contractors shall, and are required to, ensure that their employees, subcontractors, suppliers, vendors and visitors comply with the provisions of this contract.
