STATEMENT OF WORK

DBEH 16-1619, Repair Northbound S.C. Phillips Parkway, CCAFS, FL

January 28, 2019

ATCH 6 (1 of 3)

1. GENERAL DESCRIPTION:

a. Furnish all materials, labor, and equipment and perform all work required by Drawing for DBEH 16-1619, Repair Northbound S.C. Phillips Pkwy, CCAFS (24 pages) and Technical Specifications for DBEH 16-1619, Repair Northbound S.C. Phillips Pkwy, CCAFS (349 pages) to provide the Government with a complete and usable roadway shown in the said contract documents.

b. DBEH 16-1619, Repair Northbound S.C. Phillips Pkwy (aka Phillips Pkwy), CCAFS, FL – mill and overlay approximately six (6) miles, 129,500 SY of asphalt pavement roadway at CCAFS, FL to include but not limited to maintenance of traffic through work zones, restoration of traffic light loop conductors destroyed and taken out of service by the milling process, installation of new traffic and street signs, repair of grass shoulders with excavation and sod installation, and application of pavement traffic markings in accordance with MUTCD.

c. Performance Period and Paving Schedule: Performance period for this contract is 150 calendar days to include 25 work days of non-access 5 days non-access for pavement resurfacing, 25 total non-access. With the exception of weather delays, reworking rejected work, and unforeseen site conditions, contractor is to submit a construction schedule of no more than 30 calendar days for the mill and overlay of asphalt pavement road surface and application of temporary pavement markings.

2. PRIMARY AND CRITICAL WORK COMPONENTS AND TASKS

The following paragraphs highlight the primary and critical work components and tasks. These components and tasks are presented to emphasize to both the government and contractor employees the primary and critical project requirements:

a. Hot Mix Asphalt (HMA) Production and Placement, SECTION 32 12 16:

(1) Acceptance/Rejection Criteria: This is a “pay for acceptance” project. Acceptance/Rejection criteria are given in Paragraphs 1.1 – 1.1.6.

(2) Material Acceptance Testing: Testing for acceptability of the work will be performed by an independent laboratory hired by the contractor. Contractor shall submit Material Acceptance Testing as given in Paragraph 3.11 – 3.11.4.2.2 on a daily basis. Material Acceptance will be based on the following items: (1) Laboratory Air Voids, (2) In place density, (3) Grade, and (4) Smoothness.

(3) Quality Control and Quality Control Testing: In addition to Material Acceptance Testing, contractor is to provide Quality Control as given in Paragraph 3.10 – 3.10.5 and submit all QC test results on a daily basis per Paragraph 3.10.3.10, QC Monitoring.

(4) Test Section: Contractor shall initiate HMA production with construction of test section as described in Paragraph 3.5.

b. Traffic Loops: Reference drawing sheets C-101, C-113, and C-603. A traffic light and in pavement traffic loops are present at the intersection of Phillips Pkwy and Poseidon Ave. Prior to milling contractor is to submit a plan and schedule for traffic light outage, repair of traffic loops to include, mill and repaving of surrounding/affected areas, and MOT during traffic light outage. Contractor is to submit or incorporate the Traffic Light Outage Plan and Schedule with the general Maintenance of Traffic Plan described below. Additionally, contractor is to submit testing and recommissioning plan for traffic light. All work shall be performed and tested in accordance with the Traffic Detector Handbook, Federal Highway Administration, Publication No. FHWA-HRT-06-139.

c. Traffic and Street Signs: The project call to replace 11 Street Signs and 31 Traffic Signs. All signs shall meet FDOT Index 11860. To ensure accuracy, prior to fabrication and construction, contractor is to submit schedule of signs, poles and foundation dimensions for government approval. To ensure proper placement, prior to construction/dig permit request, signs locations are to be staked in field for government review. Also, from Sheet C-501: (1) All new traffic and street signs will be highly reflective. (2) Where new street signs and traffic signs are installed, all existing signs and street signs and poles are to be removed and disposed. (3) All new street signs will match existing street signs with similar font, color, and size.

d. Shoulder Repair:

(1) “Repair Shoulder Drop Off Areas”: Reference (1) call outs on Sheets C-102 – C-105; (2) Detail “Shoulder

Drop Off Repair,” shown on Sheet C-303 with accompanying table, “Repair Shoulder Drop Off Areas,” and (3) Table “Summary of Performance Turf” on Sheet C-601. These call outs, details and tables represents 355 SY of shoulder repair and installation of Bahia sod to repair areas of shoulders that are rutting or otherwise failing in need of restoration.

(2) “Shoulder Scraping”: Reference (1) General Note #4, Sheet G-002; (2) Table “Tabulation of Shoulder Scraping Quantity on Sheet C-603. This note and table represent 17,500 LF of six (6) foot wide shoulder scraping and Bahia sod installation to repair areas of shoulders that above existing pavement. Repairs are to be completed according to “Shoulder Drop Off Repair Detail,” Sheet C-303.

e. Pavement Markings: Reference Section 32 17 23, Pavement Markings, paragraph 2.21 and 2.2.9.2. Contractor shall use FS TT-P-1952, Type III traffic paint and FS TT-B-1325, Type I, Gradation A reflective media. As a component of the MOT plan, Contractor is to perform early painting of asphalt pavements in accordance to Section 32 17 23, paragraph 3.2.2. All pavement markings shown on the plans will have paint and beads applied at half the normal application rate and a second application at the normal rate 30 to 60 days after completion of the asphalt pavement surface.

f. Maintenance Of Traffic Plan:

Prior to performing work, the Contractor is to prepare and submit a Temporary Traffic Control/Maintenance of Traffic (TTC/MOT) plan to include the half rate of pavement markings and the repair of traffic loops described above. The TTC/MOT plan is to be specific to the all aspects of the project and is to be prepared by a Florida Professional Engineer with a current FDOT Advanced TTC/MOT training certificate. The TTC/MOT plan will describe/show temporary traffic controls through the work zones during active work hours and disturbed traffic areas during non-operational hours, per current FDOT Design Standards and Part 6 of the Manual on Uniform Traffic Control Devices (MUTCD). The contractor shall be responsible for the installation, removal and field maintenance of TTC devices as work progresses for all milling and resurfacing activities, grass shoulder repairs, traffic sign removal and installation, and any operation where traffic will be slowed or rerouted due to contractor operations. Personnel responsible for supervising the installation, removal, and maintenance of TTC devices shall have a current FDOT Intermediate TTC/MOT training certificate.

Any required road or lane closures must be approved by the Contracting Officer or Contracting Officer Representative prior to any road or lane closure. In general, other than Work Zone and associated MOT Zone, all portions of travel ways are to remain open; unless waived by the Contracting Officer, the milling and resurfacing operation is to be completed in accordance with FDOT Index 102-600, Sheet 9 of 12, Travel Lane Treatment for Milling or Resurfacing Notes and Detail.

Portable Changeable Message Sign (PCMS): Contractor is to provide, install, and maintain a PCMS with an approximate 6' x 11' message board at approximately Station - 5+00, on the in-bound lane/south side of SR401, two weeks prior to the start of pavement resurfacing and remain in place until paving operation is complete.

Installation of PCMS will include all work that may be required such as embankment modification/restoration necessary to provide an approximate level sign installation and acceptable roadside clearance. Messages will provide motorists with supplementary temporary control device information, updated guidance for delays, and lane closures at or near CCAFS entry control point. Contractor is to follow all FDOT Standard Plans, 600 TTC series for equipment in the FDOT right-of way and will be required to have four (4) FDOT approved plastic drums/barrels on the corners of the PCMS boards. Contractor will be responsible to request, coordinate, and obtain approval for installation and removal of the PCMS.

g. Weekly Coordination Meetings: Per specification Section 01 11 00.00 30, Paragraph 3.10.1, Contractor shall attend weekly project meetings, not to exceed 3 hours, scheduled by the Government. Contractor's Superintendent shall attend the scheduled coordination meetings. Subcontractor representatives shall attend as required. Discussion shall address the progress schedule, potential factors of delay, deficiencies, material delivery schedules, submittals, and safety issues. Prior to construction, a Traffic Safety Coordination meeting will be held with USAF Safety and Security Forces to review the contractor’s MOT plan and construction schedule. To facility initial Traffic Safety Coordination meeting, Contractor is to submit a Pavement Resurfacing Work Plan showing sequence of pavement resurfacing relative to roadway areas/ travel lanes, lane closures, traffic light outage, equipment material staging and other pertinent information.

h. Weekends and Federal Holiday Work: The government may require the contractor to work up to four Saturdays, and given government availability will approved contractor requests to work additional weekend days and federal holidays.

i. Schedule Constraints: There will be no work or lane closures on the northbound lanes from STAT 0+00 to STAT 28+00 between 6:00 AM - 8:00 AM. Similarly, there will be no work or lane closures on the southbound lanes from STAT 0+00 to STAT 28+00 from 3:00 PM to 5:00 PM. It is preferred that resurfacing from STAT 0+00 to STAT 28+00 be accomplished first and work proceed north towards the Industrial Area.

j. Submittals: In addition to the submittals listed with a “G” in column (f) on the submittal register on page 34 to 39 of the specification the Contractor is to submit the following:

(1) Maintenance of Traffic Plan and FDOT TTC/MOT training certificate as described above in Paragraph 2.f.

(2) Sign Schedule as described above in Paragraph 2.c.

(3) Traffic Light Testing and Recommissioning Plan

(4) Pavement Resurfacing Work Plan
