The following have been modified:
INSTRUCTIONS TO OFFERORS
ADDENDUM to 52.212-1 -- Instructions to Offerors -- Commercial Items (Nov 2007)
1. Submit signed and dated offers to:
45th Contracting Squadron/LGCZC
Attention: Sherri Richardson, Contracting Officer
1201 Edward H. White II, Street, MS 7200
Patrick AFB, FL 32925-3238
2. Hand carried proposals must be delivered to Bldg 423, Source Selection Room #1, Patrick AFB, FL 32925. The sealed package used to submit your proposal must show the solicitation number, time and date specified for receipt of offers, the name, address and telephone number of the offeror. Offeror’s need to submit the name, type of vehicle and tag number of the individual and vehicle that is going to be utilized for Request for Proposal to the Contracting Officer five days prior to offer delivery date for access to the base. SOME DELAY SHOULD BE ANTICIPATED WHEN HANDCARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at Patrick AFB, PRIOR to the time specified for receipt. Late proposals will be processed in accordance with 52.212-1 paragraph (f) above.
3. To ensure consideration, Offerors must follow the instructions contained herein. The response shall consist of two (2) separate volumes; Volume I – Present and Past Performance and Volume II – Price/Costntract Information.
4. Volumes and Binding. Offers will consist of submitting two (3) separate proposal volumes: (I) Present/Past Performance, (II) Cost/Price and Proposal Documentation in the number of copies and page count specified below. Failure to abide by the guidelines set forth below may result in rejection of the applicable proposal volumes with adverse consequential effects.
4.1. Each volume of the proposal must:
- Be separately bound in a standard three-ring loose-leaf binder, permitting the volume to lie flat when open and allow for easy removal and replacement of pages. (excluding “original” copy).
- Staples shall not be used.
- The cover and spine of each binder will clearly identify the Offeror’s name, volume number, RFP number, RFP title (e.g., Company A, Vol I, RFP FA2521-07-R-8507, Refuse Collection and Disposal Services) and copy number (e.g. copy 2 of 4).
- The original copy should be clearly marked “original” and shall be 2-hole punched at the top center of each page and should be packaged but not bound.
4.2. Electronic Copies. One complete copy of the proposal shall be submitted in an electronic format:
- Offerors shall use Microsoft Windows-compatible, virus-free CD-ROM (CD-R media is acceptable).
- The electronic copy of the proposal shall be submitted in Adobe Acrobat (version 7.0 compatible).
Exception: Microsoft Excel (Office 2000 compatible) shall be used for any cost/price spreadsheets.
Formulas used to calculate total costs must be reflected in all spreadsheets in the cost/price and proposal document volume. All proposal documents and cost/price information must be placed on the CD.
- Replacement disks will be required to update the final proposal resulting from any discussions and updates.
- Ensure the electronic documents stored on CD can be opened.
- Volume II, Cost/Price and Proposal Documentation, Copy I, submit one read-only copy. Care should be taken to avoid optional settings in the Excel spreadsheets, which prolong analysis of the spreadsheets; e.g., use of settings in Excel such as manual calculations are not desirable.
- Indicate on each CD the volume number and title. Use separate files and folders to permit rapid location of all portions, including exhibits, annexes, and attachments, if any.
- The printed proposal and electronic proposal submissions must be identical. If there is a variancebetween the electronic copy and the hard copy, the hard copy version will take precedence.
4.3. Pages and Text. Page size will be 8.5 x 11 inches, not including foldouts (graphics), with at least 1-inch margins around the page perimeter. A page is defined as one face of a sheet of paper containing information.
- Pages will be single-spaced.
- Two pages may be printed on one sheet of paper, front and back.
- Partially used pages will count as an entire page.
- The background color of each page of the original proposal submission shall be white stock only.
- Use font Times New Roman or Arial, with a font no smaller than 11 point with normal proportional spacing. No character spacing modifications (kerning) or line spacing modifications to the standard fonts are permitted.
4.4. Charts and Tables: Foldouts used for charts, graphics, tables, exhibits or diagrams may not exceed 11 x 17 inches. All information (except for document numbers, page numbers, etc.) must be contained within an image area of 9 x 15 1/2-inches. Text used for callouts on graphics and exhibits shall be legible and no smaller than font Times New Roman or Arial, 11 point. For page count purposes, each printed side of foldout page shall count as two pages.
4.5. Table of Contents, Indexing and Glossary:
- Each volume shall contain a detailed table of contents to delineate the subparagraphs within that volume.
- Tab indexing shall be used to identify sections.
- Each volume shall contain a glossary of all abbreviations and acronyms used, with an explanation for each. However, use of abbreviations and acronyms should be extremely limited.
- Table of Contents and Glossaries do not count against the page limitations for their respective volumes.
4.6. Change pages after initial proposal submission:
- If the Offeror submits proposal revisions any changed pages that are submitted shall be a different color than the original proposal pages as specified by the contracting officer.
- Changed information clearly marked by a vertical line in the right margin of the page identifying the changed content.
- The date of the revision and revision number shall be reflected on each changed page (preferably on, or next to, the vertical change line).
- Spreadsheet revisions obvious by highlighting source cells that changed.
- The offeror shall also provide a tracking list of all changes with references to the volume, page, and paragraph number.
5. RECEIPT OF PROPOSALS. The Offeror’s Volume I – Present/Past Performance and Volume II – Price/Cost, paper and electronic, shall be delivered to Ms. Sherri L. Richardson as referenced in paragraphs 1 and 2 above. If using a courier, submit the name of the individual hand-carrying this volume 72-hours in advance to the CO. The courier will be required to provide identification at the time of delivery. All proposals must be received no later than 2:00 p.m. Eastern Time (ET) on 21 March 2008. Failure to submit entire proposal by the date and time specified above will be handled as a late proposal in accordance with FAR 52.215-1(c) (3) (ii) (A).
A. VOLUME I – PRESENT/PAST PERFORMANCE (FACTOR 1) - Submit original and four (4) hard copies and one (1) electronic (soft) copy. (45 page limitation)
(1) This portion of the evaluation will consider the Offeror's performance including prime contract and subcontractor experience, and the Offeror’s proposed subcontractors, teaming partners, key personnel and predecessor companies.
(2) Provide not more than six (6) recent and relevant performance citations for the Prime Team (includes citations for both prime and subcontractors) in the format requested below. Submit present/past performance information in accordance with the format and instructions contained in Attachment 212-1-4 on all efforts included in the Relevancy Matrix (Attachment 212-1-3).
(3) Relevancy. Relevant means the performance being considered must have logical connection to the work, described in the solicitation. Normally, relevance would include such aspects as the product or service similarity, product or service complexity, contract type, program or lifecycle phase, contract environment, division of company proposing, subcontractor interaction and magnitude. Offeror must explain why each past performance citation is relevant to this acquisition. Identify the role of each proposed subcontractor, teaming partner and key personnel to permit the evaluation team to appropriately attribute relevance to the past performance.
(4) Organizational Structure Change History. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what present/past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a “roadmap” describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for present/past performance evaluation/performance confidence assessment. Since the Government intends to consider present and present/past performance information provided by other sources as well as that provided by the Offeror(s), your “roadmap” should be both specifically applicable to the efforts you identify and general enough to apply to efforts to which the Government receives information from other sources. Include Commercial and Government Entity (CAGE) codes for any predecessor companies to facilitate Government research
(5) Consent Letters. Consent letters executed by each joint venture partner and/or subcontractor, authorizing release of adverse present/past performance information so the Offeror can respond to such information. A sample consent letter is in Attachment 212-1-1
(6) Client Authorization Letters. Client authorization letters for each identified effort for a commercial customer authorizing release to the Government of requested information on the Offeror’s performance (see sample letter at Attachment 212-1-2).
(7) Relevancy Matrix. Provide a completed relevancy matrix including each teaming contractor and/or joint venture partner and proposed subcontractor (see format in Attachment 212-1-3). Do not identify present/past performance for proposed team members in areas they are not proposed to perform.
(8) Questionnaires. Offerors are responsible for sending out Past Performance Questionnaires, Attachment 212-1-6 for each citation. Completed questionnaires should be submitted directly to the Contracting Officer by 21Mar 08. Once the Past Performance Questionnaires are completed, the information contained therein shall be considered sensitive and shall not be released to you, the Offeror. A sample cover letter for transmitting the questionnaire is provided at Attachment 212-1-5. Questionnaire recipients (Customer POC’s) shall be listed as required in Attachment 212-1-6. Offeror shall verify the POC information is current and valid.
(9) Contractor Performance Assessment Reports (CPARs). Offerors are requested to indicate in the Present/Past Performance Volume whether this information exists (and in which database it resides) and are encouraged to provide additional information regarding the work effort to clearly describe how it relates to the PWS requirements. If a CPARS or other evaluation form has been accomplished on the Offeror’s existing work, but has not yet been posted to an automated system, the Offeror is requested to advise the CO and provide a point of contact and phone number from which the information may be obtained. This information should be provided to the CO at the same time the Present/Past Performance Volume is submitted. Also include information on cited contracts that have been captured in a database other than those used by DoD or other federal agencies.
B. VOLUME II – PRICE/COST (FACTOR 2) - Submit original and three (3) hard copies and one (1) electronic (soft) copy. No page limitation.
(1) Complete blocks 12, 17a, 17b and 30a, b, and c of the SF 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the SOLICITATION, with attachments. The SOLICITATION constitutes the model contract.
(2) Insert proposed unit and extended prices in Section B, Price/Cost Schedule, to include unit prices in Section B, Price Table for all years. The proposal(s) must be submitted for the base year plus all four (4) Option Years.
(3) Complete all required and necessary fill-ins and certifications in provisions and clauses identified in Sections C through K.
4. Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 180 calendar days from the date specified for receipt of offers.
5. Product samples are not required.
6. Data Universal Numbering System (DUNS) Number applies.
