MEMORANDUM FOR AFNWC/PZIC

ATTENTION: SSGT JARRETT J. SAINZ

FROM: 377 MSG/CEOE

SUBJECT: F2K2AC4066A003 Well #20 Repair

A site tour was conducted on May 5, 2014 for the repair of well #20. The following are the questions and answers from the site tour.

1. Are the lifting eyes for roof removal attached to roof “I” beams?

a. Yes. The lifting eyes are constructed as shown on page 16 of the Performance

Work Statement.

2. What is required of containing the oil that may still be in the bearing drip line?

a. Oil shall be treated as required in section 1.11 of the PWS. Additionally oil treatment will be contained and treated as approved in the contractors spill prevention plan. The Material Safety Data Sheet for the oil will be provided to the successful bidder.

3. Will the work need to be complete within 30 days?

a. The work will be completed as described in section 2.2 of the Performance Work

Statement. “The contractor shall complete the diagnosis/investigations of the

Well within 23 calendar days from the notice to proceed. The contractor will provide a schedule for the repairs, however, it is expected that the repairs will be completed within 30 days of completing the diagnosis. The repairs performed by the contractor shall be approved by the government before commencing on repairs.”

4. Will there be inspection and government representative?

a. The contracting office shall appoint a government representative and there will be government Quality Assurance and Quality Control. Any and all contractual changes, modifications, will be performed only by the contracting officer.

5. Is there a need for a test pump?

a. No. The existing pump will suffice for the testing required in the PWS.

6. How many performance information forms are required?

a. In “1.Adden._52_212-1,3.i(a).,PAST PERFORMANCE INFORMATION” page

2, replace “Provide no more than three (3)” with ” Provide no more than five (5)”

7. Is a NM license with an EE-98 required for the motor removal and installation?

a. According to NM statute 14.6.6.10 EE-98 is for residential and commercial wiring 5,000 or less volts however it does not include ES-10. The ES-10 is specific to water well pump installers. The contractor shall have a New Mexico license with an EE-98 and an ES-10.
