ADDENDUM FAR 52.212-1
ADDENDUM TO FAR 52.212-1
INSTRUCTIONS TO OFFERORS (ITO) – COMERCIAL ITEMS
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of three (3) separate parts; Part I - Price Proposal, Part
II - Technical Proposal, and Part III - Past Performance Information.
B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.
C. Specific Instructions:
1. PART I – PRICE PROPOSAL - Submit one (1) original (labeled as such), one (1) copy and one (1) electronic copy on compact disc.
(a) The offeror must ensure they submit all required documents stated in the solicitation.
In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation.
(b) Offeror shall provide a completed proposal form (see Attachment 5 to RFP) in a pdf format and excel format for proposal prices to include all option periods, as applicable. The extended amount must equal the whole dollar unit price multiplied by the number of units. The contractor shall include unit prices for the individual lump sum items 1-8 and 17-26. The contractor shall include their labor rate for items 9-16. The contractor shall include a not to exceed price based on the information provided in the solicitation.
(c) The offeror shall submit required certifications in its entirety and only those pages that require fill-ins as required by the solicitation.
(d) If applicable, provide a copy of the letter from the Small Business Administration
(SBA) showing proof of Section 8(a) status.

2. PART II – TECHNICAL PROPOSAL - Submit one (1) original (labeled as such), three (3) copies and one (1) electronic copy on compact disc. Limited to no more than 20 pages per hard copy.
Offeror shall:
(a) Subfactor 1: Provide a Technical Approach Narrative that indicates a clear understanding of the Statement of Work (SOW) by summarizing the technical approach the
Offeror intends to employ during the construction of subject contract. This narrative shall include the Offeror’s methodology that will be utilized during mobilization, scheduling, management of work and execution of the work outlined in the SOW provided. This narrative shall also include the Offeror’s Quality Control structure and objectives as it is applied internally as well as how it is applied for oversight of sub-contractor work and performance. This part shall include the offeror’s Quality Control Plan, a statement indicating what personnel will be used for this function on subject project and any deficiency tracking mechanisms employed by the offeror.
(b) Subfactor 2: Provide a Proposed Progress Schedule for anticipated
Diagnosis/Repair activities in Microsoft Project or similar format. The proposed progress schedule shall clearly show the activities and logical sequencing of tasks required to successfully complete the contract requirements defined in the SOW. Demonstrate a clear understanding of the entire project, the durations for each phase and the prime contractor and sub-contractor resources necessary to execute each phase, as well as the overall anticipated performance period.
3. PART III – PAST PERFORMANCE INFORMATION - Submit (1) original (labeled as such) and three (3) copies. Limited to no more than 1 page per contract listed.
Offeror shall:
i. (a) Provide no more than three (3) completed Past Performance Information forms.
(Attachment 6 to the solicitation) Projects chosen shall be recent and relevant as defined below. All blocks on the Past Performance Information form must be completed and all data must be accurate, current and complete, to include the DUNS number and two (2) Points of Contacts/References for each project. The Offeror is responsible for validation of contact information for the Points of Contact/References.
RELEVANT Past Performance is defined as performance on projects that are similar in size, scope, complexity and dollar magnitude as the project described in this solicitation. For dollar magnitude, similar will be defined as a range from 50% to 100% of subject project’s estimated contract value.
RECENT Past Performance is defined as performance on projects that are 100% complete and were completed no more than three (3) years from the date of this solicitation.
NOTE: The Government will consider past performance information only on projects that are 100% complete.
(b) If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past relevant contracts.
(c) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
D. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:
1. Requirements of the RFP (CLINs & SOW) and government standards and regulations pertaining to the SOW.
2. Evaluation Factors for Award in Addendum to FAR 52.212-2 (Attachment 2) of this RFP.
3. Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in this Addendum to FAR 52.212-1 (Attachment 1) will not be read or evaluated, and will be removed from the proposal.
4. Format for proposal Parts II and III shall be as follows:
(a) The proposals will be 8 1/2” x 11” paper except for fold-outs used for charts, tables, or diagrams, which may not exceed 11” x 17”.
(b) A page is defined as one face of a sheet of paper containing information.
(c) Typing shall not be less than 12 pitch.
(d) Elaborate formats, bindings or color presentations are not desired or required.
