1.0 SCOPE

The contractor shall troubleshoot/diagnose, repair well, reinstall removed equipment, adjust/set well pump and motor, test, disinfect and monitor Well #20(building 20437). The contractor shall provide all services required to repair Well #20. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to affect the repair.

1.1 History/Background

Well #20 was constructed in 2007 and has been in operation since July 2008. Well #20 is estimated to have approximately 19,143 hours based on water production design rate of 1,400 gallons per minute (actual 1550 gpm). The motor on Well #20 had some burned up windings and was replaced on October 15, 2013. The breaker on the Motor Control Center was open and the well failed to operate. Maintenance crews discovered the issue and tried to restart the motor in manual operation. The motor did not start and it was noticed by maintenance crews that there were pieces of copper at the bottom of the motor. The damaged motor was replaced. The new motor was set in place and preliminarily adjusted by setting the nut at the top to the same place as the damaged motor. The new motor was operated in bypass and the electrical connections checked to verify rotation and balance. There was no indication by checking voltage/amps that the motor was overloaded. There was noise noticed when the pump was shut down after testing.

The pump was adjusted and there is still noise from an unknown source that appears to be coming from an area lower than the motor and pump head. The pump appears to be bound up and will not rotate manually.

1.2 Applicable Codes/Regulations

The Disinfection shall conform to AWWA A100-06 (Water Wells), AWWA B301-10 (Liquid

Chlorine), AWWA C654-13 (Disinfection of Wells) and New Mexico Environmental

Department (NMENVD). All construction type activities shall comply with OSHA and AFI regulations.

1.3 Normal Hours of Operation

The contractor shall perform the services required under this contract during the following hours:

7:00 AM to 4:00 PM, Monday through Friday, except on federal government holidays.

1.4 Contractor Support

The contractor shall provide a Primary and Alternate Point of Contact (POC) that shall be responsible for ensuring that the contract services are provided satisfactorily. The Primary and

Alternate POCs must be able to read, write, speak, and understand English and have the authority to act as a binding agent in regards to this contract. The name of this person and an alternate (or alternates), shall act for the contractor when the POC is absent, shall be designated in writing to the Air Force Nuclear Weapons Center Contracting Officer and the 377th Civil Engineer

Division Quality Assurance Personnel at the start of the contract and for the duration of the contract.

1.5 Contractor Employees

The government reserves the right to restrict the employment, under this contract, of any contractor employee, or prospective contractor employee who is identified as a potential threat to health, safety, security, general well-being, or the operational mission of the installation and its population. The contractor shall employ competent and qualified supervisory, administrative and direct labor personnel to accomplish the work performed under this contract.

1.6 Physical Security

The contractor shall be responsible for safeguarding all government property within the confines of the well #20 fenced area. At the close of each work period, government facilities, equipment, and materials shall be secured. The contractor will be required to have a pass to obtain access to

Kirtland AFB.

1.7 Storage of Equipment and Supplies

The contractor may store removed equipment and items at the work site. The contractor will be responsible for security of its equipment. Equipment may be stored at the Well #20 yard.

Attached is a plan view of the yard and location of Well #20. The Well #20 yard is approximately 127 x 242 feet squared with chain link fencing and large double gates.

1.8 Excavation and Backfilling

1.8.1 Excavation Permit.

The Contractor shall obtain an excavation permit from the base civil engineer or a designated representative before commencing any digging or excavation on the Base. The Contractor shall obtain a Base Civil Engineering Work Clearance Request, AF Form 103, prior to any trenching or digging and a Base Civil Engineer Work Request, AF Form 332, prior to any facility, man-hole, or hand-hole modification. Trenching, excavation, confined spaces entry, confined spaces atmospheric testing/forced air ventilation, marking and barricading of open trenches are to be performed IAW Occupational Safety and Health Act (OSHA) standards, any local procedures, and other provisions of the PWS as listed.

1.8.2 Excavation Methods.

Hand digging is required within four (4) feet of existing utilities identified by excavation permit and in areas immediately adjacent to and within approximately one (1) foot of buildings.

1.8.3 Backfilling

Backfilling shall be accomplished with well-pulverized soil and each twelve (12) inch layer of soil shall be compacted to a CE 55 density of at least ninety-five (95) percent.

1.8.4 Open Excavation

Open Excavation shall be barricaded with appropriate safety barricades when Contractor personnel are not present in the immediate vicinity of the work site.

1.9 Final Cleanup

After the work is completed, the work site shall be returned to its original state.

Contractor will replace or repair all existing adjacent finished surfaces, utilities, equipment, products, vegetation, and structures or parts thereof that have been damaged or removed to perform the work.

1.10 Disposal of Waste

Disposal of Waste shall be performed appropriately. All non- hazardous waste may be disposed in the KAFB waste disposal area as approved by the government. The government will not furnish any equipment or provide any support for waste disposal.

1.11 Hazardous Materials

Hazardous Materials that will be transported into KAFB shall be approved by 377 MSG/CEIE.

The contractor shall have an MSDS for all hazardous materials within their possession. Any chemicals in excess of 1 gallon shall have a spill prevention plan (SPILL PREVENTION

PLAN). Hazardous materials entering KAFB shall be transported appropriately and any stored flammables shall be stored in an approved flammable cabinet. Any waste streams created from hazardous material used by the contractor shall be removed from the KAFB and disposed of appropriately.

Ensure equipment and vehicles do not contaminate the ground due to leaks of contaminating fluids.

2.0 Project Administration

The government shall have a representative Point of Contact responsible for coordination with the contractor. The contractor shall make available its supervision for Quality Assurance and

Quality Control. Government representatives may, at any point, request access to the work location to ensure quality and verify that requirements are being met.

2.1 Administrative Records

All records, documents, and associated papers provided by the government and generated during the period of this contract become government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review by the Government. The contractor shall supply to the government any operations and maintenance manuals associated with any parts that are replaced and including part numbers, sizes, type and other pertinent information that may be necessary to repair or replace the parts in the future.

2.2 Schedule

The contractor shall complete the diagnosis/investigations of the Well within 23 calendar days from the notice to proceed. The contractor will provide a schedule for the repairs, however, it is expected that the repairs will be completed within 30 days of completing the diagnosis. The repairs performed by the contractor shall be approved by the government before commencing on repairs.

3.0 Tasks and Technical Requirements

3.1 Equipment and Supplies

The contractor will be responsible to supply all equipment and materials required to perform the repair. Equipment entering and used at Kirtland AFB shall be inspected and be in good mechanical condition while on the AFB. The access to the well is via paved streets and located on Griffin Ave and Texas Street. Large truck access is via Hickman Street, at the Contractor

Gate, approximately 5 miles from the well site. The contractor equipment that may contact the well water shall be cleaned to prevent contamination of well #20. All debris, oils, and other contaminants shall be removed from equipment and equipment disinfected before contacting the well.

3.2 Well Information

Well #20 is a 44 inch diameter hole well with a 36 inch diameter casing approximately 39 feet from the surface and then a 33 inch borehole with 20 ¾ inch outside diameter casing to a depth of approximately 680 feet from surface to the bottom of the pump bowl.

3.3 Pump Information

The existing pump is manufactured by National Pump Company (SN: 120750) and is a vertical turbine line shaft pump with 12 stages. The pump rating is 1,400 gallons per minute at a total design head of 736 feet of water column. The pump assembly is approximately 50,000 pounds

(lbs) (46,327 lbs) which includes the motor. The pump has centering spiders approximately every 40 feet. There are approximately 134 tube bearings.

3.4 Investigations and Diagnosis

The contractor will identify/diagnose the required repairs for Well #20. A video log may be required to determine “as is” condition. All equipment must be disinfected before it is used in the well per AWWA standards. Equipment and parts will need to be removed as part of the investigation. All parts and equipment removed shall be inspected, cleaned, labeled and stored appropriately. If parts are determined to be damaged, the contractor will advise the government of the level of damage. The contractor will provide to the government written recommendations for repair of the well and the cost associated with the repair. The government will review the recommendations and negotiate with the contractor on the repair of the well.

The existing Well has a metal building with a metal roof covering the Well which may need to be removed to perform the investigations and repair. There are no existing hatches in the roof to access the equipment. The panels from the building may be removed as well as the roof and shall be reinstalled back to the pre repair condition. The roof including the structural support is approximately 3,025 lbs. Lifting hooks located at each corner are rated at 1,000 lbs each as shown in the drawing in Appendix B.

Based on the investigation the contractor may recommend replacement of parts and equipment that may not be damaged but may have reached its usable life. The government will review recommendations prior to repairs, approve some or all recommendations, and contractor will only perform the government approved recommendations.

3.5 Repairs

Repairs on Well #20 will commence once the contractor has determined through investigation/diagnosis the necessary course of action required to repair the Well. The contractor shall present to the government a written plan/schedule for accomplishing the repairs; this plan is due within 2 days of completing the investigations.

The repairs may require removing/reinstalling the motor, pulling the line shaft for examination/repair, removing bearings and examination, removing the centering spiders and examining, replacing the airlines, remove/replace tube/shaft bearings, replacing the water Pilot transducer, removing/replacing pump bowls, removing/replacing impellers, remove/replace suction bearings, disinfection of well, well development, and testing the well to provide pre repair flow rate of 1,550 gallons per minute.

The water pilot down hole level transducer requires replacement. There is a ¼ inch diameter airline that is damaged and requires replacement.

The contractor shall provide new parts manufactured by the same manufacturer as the replaced parts or “equal”. The contractor will provide to the government representative the specifications and warranty information for the as “equal” parts and reason for deviation from the existing manufactured part.

4.0 Contractor Requirements

4.1 Clarification of Submittals on Form 66

The contractor will supply the following documents to the government representative and as shown in AF 66.

The contractor will provide to the government the recommendations for repair of the Well

(REPAIR RECOMMENDATIONS). Included with the recommendations the contractor will provide to the government a detailed cost estimate for the recommended repairs using the repair cost estimate breakdown provided in the solicitation. A detailed repair schedule (REPAIR

SCHEDULE) shall be provided to the government after the diagnosis is complete.

The contractor will submit the plan (DISSINFECTION PLAN) for disinfection of the well and any test reports associated with the disinfection.

The contractor will provide to the government a written description of the repairs performed

(REPAIRS DOCUMENT) and any recommendations for improvements or required maintenance necessary to ensure the Well continues to operate. As part of the documentation the contractor will provide a revised pump curve for the repaired/new pump. The contractor will provide quantities and types of chemicals added to the well for acidizing/chlorinating/phosphate additives.

The contractor shall submit operations and maintenance manuals (O&M MANUALS) for new parts installed as part of the repair.

5.0 Entry/Escort Service.

The government point of contact will arrange for entry and escort service if needed for Kirtland

Air Force, when required.

5.1 Points of Contact

5.2 Facilities

There is 120 Volt power located at the Well and control room available for use by the contractor.

Others utilities may be made available by the government given sufficient notice of 24 hours by the contractor.

6.0 Documents Incorporated by Reference

The contractor shall comply with the standards and requirements set forth in the regulations and manuals pursuant to the industry they are supplying the services for. A copy of DOD and Air

Force publications will be made available for the contractor's review upon request to the

Contracting Officer.
