The affidavit pertains to ITN #26-0059 for an Applicant Tracking System and requires a vendor's authorized representative to certify under penalty of perjury that the company does not use coercion for labor or services as defined in Section 787.06(13), Florida Statutes. The certification covers seven specific prohibited practices: use or threat of physical force; unlawful restraint, isolation, or confinement; debt bondage through labor; destruction or withholding of immigration or government identification documents; threats of financial harm; fraud or deceit in recruiting; and provision of controlled substances for exploitation. The form requires the vendor's name, Federal Employer Identification Number (FEIN), authorized representative name and title, complete address, phone number, and email address, along with the authorized representative's signature, printed name and title, and date of certification.

This affidavit is a mandatory compliance document that vendors must complete and submit as part of their proposal package for the Sarasota County School Board's Applicant Tracking System procurement. The form has been approved by the General Counsel as of April 24, 2024, and serves as a prerequisite certification for contract award consideration. There are no pricing, set-aside programs, or funding requirements associated with this specific form, as it functions solely as a legal compliance and attestation document to ensure vendor integrity and adherence to Florida labor standards.
