Management of Leased Space assessment includes lessor's provision of experienced staff, building and grounds maintenance, and delivery of services such as janitorial, snow removal, utilities, and repairs. Customer Satisfaction evaluates construction quality, responsiveness to service requests and communications, issue resolution, and professional conduct. Timeliness measures adherence to lease agreement schedules, space delivery timeliness, punch list completion, and service promptness. The survey includes sections for explanatory comments, narrative summaries addressing lessor strengths, improvement opportunities, compliance difficulties, future solicitation reservations, and additional performance notes. For government leases, evaluators must disclose any terminations for default or convenience and pending terminations. The form requires the evaluator's printed name, signature, company name, phone number, date, lease name, and period of performance.
