About this file
This is a sample Past Performance Questionnaire template designed for the Department of Veterans Affairs to evaluate contractor performance on federal contracts. The questionnaire is intended to be distributed to individuals familiar with a contractor's day-to-day operations and service delivery to solicit candid feedback on performance quality. Completed responses must be submitted to brittany.rodriguez7@va.gov by a deadline to be specified by the VA, with confidentiality of respondent identities maintained per FAR 15.306(e)(4).
The questionnaire evaluates contractors across six key performance categories using a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory). Quality of Work addresses compliance with lease requirements, project completion standards, maintenance issues, safety compliance, and contractor selection likelihood. Response Timing assesses timeliness in correcting deficiencies and submitting required documentation. Cost Control evaluates the original contract amount, deductions for unperformed work, and cost management effectiveness. Business Management rates on-site personnel management, quality control, and subcontractor coordination. The questionnaire also includes sections for Overall Contractor Rating and detailed comments throughout. This template applies to the related contract opportunity RLP 36C24825R0137, which appears to involve facility maintenance or real property services for the VA.
