About this file
This is a General Notes Supplement document issued by the Volusia County Department of Public Works, Road & Bridge Division for the Main Street Bridge (Bridge No. 794004) Deck Repair Project. The project involves replacing full-depth concrete on the eastbound deck, sidewalk, and railing on Span 47 of the bridge over the Halifax River. The work encompasses all labor, materials, equipment, removal and disposal of materials, traffic control, functional checks, operation and maintenance manuals, and as-built surveys and drawings necessary for complete project installation. The contract is structured as a lump sum agreement where the contractor assumes full responsibility for estimating all work quantities and materials required. Payment will be made on a percentage-complete basis as progress is estimated by the contractor and approved by the county, with adjustments to the accepted Schedule of Values allowed only through Change Orders.
The document specifies that the lump sum price bid shall fully compensate the contractor for all work completed and ready for continuous operation and use. No separate payment will be made for applications and pulling of permits, shop drawings, preconstruction documentation, survey and construction layout, field engineering, dewatering, trench excavation and shoring, backfill and grading, testing, cleanup, utility maintenance, or closeout documentation; all such costs must be included in the appropriate pay items. Measurement of work is defined using standard units including linear feet, square feet, cubic yards, tons, and lump sum categories. All work under the contract must be included in the bid schedule, and any items not shown or omitted that are required for complete installation shall be furnished and installed by the contractor at no additional cost to the owner. The contract is subject to retainage as defined in the General Conditions and the Agreement.
