About this file
This document is a Custodial Evaluation Form from West Virginia University (WVU) designed for systematically assessing the performance of contracted custodial laborers. The form is to be completed by a WVU Lead or Supervisor one month after a laborer's first scheduled work day, and then every two months thereafter until contract termination. The evaluation covers multiple performance dimensions including demeanor, responsibility, initiative, work proficiency, and attendance across categories such as general laborer conduct, custodial-specific tasks, and low-wage laborer expectations.
The evaluation uses a numeric rating system from 1-5 (with N/A as an option), where 1 represents "Exceptional" performance that significantly exceeds position requirements, and 4 represents "Unsatisfactory with Insufficient Progress Over Time". The form provides detailed criteria for assessment, such as communication effectiveness, professional boundaries, task completion, safety awareness, chemical and equipment usage, and overall reliability. Attendance and punctuality are also explicitly tracked, with specific numeric ratings for absences and late arrivals. The primary purpose of the document is to confirm job expectations, provide objective feedback, and establish potential corrective improvement plans for contracted custodial personnel.
