About this file

This is an Exhibit G document detailing the Certificate of Final Recycling requirements for a state or local recycling contract. The document establishes the certification procedures and documentation requirements that the Subcontractor must provide for each batch of recyclable material processed under the contract terms. Each certification must be submitted within 60 days of material pickup and must include the pickup date, pickup location, description and lot number of recyclable materials, confirmation that processing complied with the approved Comprehensive Recycling Plan, and verification that all facilities used met applicable environmental regulations. The Subcontractor Authorized Representative must sign each certificate, and no payment will be released to the Subcontractor until all certifications for invoiced services have been received and approved by the Government.

The document provides a standardized certification form template that Subcontractors may use, or they may submit an alternative form if it has been approved in writing by the Government. The certification must confirm that all recyclable materials and byproducts have been transported, stored, and recycled in accordance with the submitted Recycling Plan and that all processing facilities have maintained compliance with applicable environmental regulations. The form requires specific fields for pickup date, lot or delivery order number, pickup location, amount recycled, Subcontractor firm name, authorized representative signature, and date signed.
