About this file

This file is a Foreign Country of Concern Attestation form (PUR 1355) required by the State of Florida for entities bidding on or entering into contracts with governmental entities that would grant access to Personal Identifying Information. The form mandates that bidding entities attest under penalties of perjury that they are not owned by, organized under the laws of, or have their Principal Place of Business in a Foreign Country of Concern, and that no foreign government of concern maintains a controlling interest in the entity. The attestation must be completed and signed by an officer or representative of the submitting entity and includes fields for printed name, title, signature, and date. This requirement applies to all bids, proposals, replies, and contract renewals or extensions involving access to individuals' Personal Identifying Information under Florida Administrative Code Rule 60A-1.020.

No pricing terms, set asides, incumbents, funding sources, or contract-specific financial information is contained within this attestation form, as it serves solely as a compliance and eligibility certification document. The form functions as a prerequisite condition for participation in Florida government contracting opportunities where personal data access is involved, rather than as a substantive solicitation document addressing service scope, deliverables, or compensation structures.
