About this file

This document is an exhibit from the Arizona Department of Community Services (DCS) Office of Real Estate providing contractor notification requirements regarding asbestos-containing building materials (ACBM) and presumed asbestos-containing materials (PACM) for janitorial services contracts. The exhibit establishes mandatory safety protocols that contractors and their employees must follow when working in DCS facilities. Contractors are required to acknowledge their understanding that certain DCS facilities contain ACBM or PACM as documented in available asbestos surveys, and they must commit to never disturbing these materials during service delivery. In the event that such materials are accidentally disturbed or other suspicious materials are discovered, contractors must immediately cease work and contact DCS Risk Management at (602) 542-5698. All activities conducted within DCS facilities must comply with applicable state and federal guidelines including OSHA and EPA regulations (29 CFR 1910.1001, 29 CFR 1926.1101, and 40 CFR 763.92).

The document requires supervisory contractor representatives to sign the notification form acknowledging their responsibilities, and it mandates verification of worker asbestos awareness training documentation by a designated party prior to contract performance. This exhibit serves as a binding acknowledgment of contractor compliance with asbestos handling protocols and represents a critical safety requirement for the janitorial services opportunity in Show Low, where proposals were due by August 12, 2025.
