About this file

This is a Departmental Policy/Procedure document issued by the City of Phoenix Public Transit Department (PTD) establishing protocols for reporting, documenting, and coordinating repairs to PTD facilities, equipment, and assets. The procedure applies to all PTD staff, the Police Transit Unit, contractors, tenants, and vendors operating at PTD facilities, including Transit Department Headquarters, Transit Centers, Park-and-Ride locations, and North, South, and West Operating Facilities. Any individual discovering or causing damage to City property must complete the Public Transit Department Incident/Repair Service Request Form, notify their supervisor immediately, contact on-site security if applicable, and notify the City Contract Manager responsible for their contract. All incident forms must be complete, signed, and submitted with photographs of damage and copies of quotes or estimates. The procedure became effective on April 22, 2026, and represents Revision Number 4 of this policy.

The document establishes that the PTD Facilities Division will coordinate all repairs through the PTD Facilities Maintenance Contractor, Facilities Cleaning Contractor, other City departments, or City-contracted vendors, with all repairs following City purchasing guidelines and Original Equipment Manufacturer specifications. Contractors are financially responsible for damage resulting from misuse, abuse, neglect, accidents, improper operation, or theft by their employees, subcontractors, or visitors, and repair costs are charged to the responsible contractor. Contractors may submit disputes in writing via email to the City Contract Manager within five business days of receiving the Incident Report, with the PTD Facilities Division providing a written response within five business days of receipt. Invoicing for recovery costs is managed by the PTD Management Services Division and issued after the City processes payment for completed repairs. Emergency contact information is provided for after-hours incidents, with normal business hours defined as 7:00 a.m. to 4:00 p.m., Monday through Friday, excluding City holidays, and life-threatening situations or widespread property damage requiring immediate calls to 911.
