About this file
This is a Subcontractor Information and Consent Form (Exhibit B) for federal contract proposal 36C25626R0018 related to Project No. 586-25-101 SITE PREP IR UPGRADE. The form serves as a template that prime contractors must obtain from major subcontractors and teaming partners participating in their proposal response.
The form requires subcontractors and teaming partners to provide consent allowing the Government to disclose their past and present performance information to the prime contractor during the source selection process. Subcontractors must complete the form with their firm name, contact information, DUNS/CAGE/SAM identifiers, and the name and details of the prime contractor they are partnering with. The form also requires submission of a reference list containing at least three contract examples, including contract numbers and titles, descriptions of the subcontractor's role (prime or sub) and work performed, and reference points of contact with email addresses and phone numbers. The completed consent forms are to be submitted by the offering contractor as part of the past performance volume submission to facilitate the Government's assessment of subcontractor past performance relevancy in evaluating proposals for best value.

EXHIBIT B – SUBCONTRACTOR INFORMATION AND CONSENT FORM
Subcontractor and/or Teaming Partner Consent for the Release of Past Performance Information to the Prime Contractor and Reference Information
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government in assessing your past performance relevancy we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted to the offering contractor for submission with past performance volume.
