About this file

This document is Standard Form 30 Amendment 0002 to a Request for Information (RFI) issued by the Department of Veterans Affairs, Network Contracting Office 16 in Houston, Texas, for solicitation 36C25626R0018 dated January 27, 2026. The amendment provides RFI responses related to site preparation for an infrared (IR) and computed tomography (CT) equipment installation project (586-25-101).

The RFI responses address 15 key questions regarding project execution and site conditions. Notable responses include: (1) a single qualified superintendent can fulfill superintendent, quality control, and safety/site health officer roles if properly certified; (2) the project will require a minimum of two phases, possibly three, to be determined during the design phase; (3) no temporary systems (HVAC, generators, power) are required; (4) the contractor coordinates equipment installation and removal, with equipment becoming the manufacturer's property for salvage; (5) an oversized sliding door on the east side provides access for equipment removal; (6) magnetic and radiation shielding are not currently in place and must be installed; (7) existing equipment includes Philips CT Ingenuity System (728326, SN 320476) and IR Allura XPER FD 20 (Module 000444, SN 000607); (8) a crawl space exists beneath the work area in the sub-basement; (9) the original building's structural design supports 100 lbs. PSF, with load distribution requirements to be determined by the design-build team's structural engineer; (10) the VA Radiation Safety Officer will provide shielding design requirements; and (11) project engineers assigned to the architectural-engineering team must be licensed professionals.

1. What on-site personnel will be required by the Prime Contractor? Superintendent, Quality Control, SSHO?

RESPONSE: All 3 will be required and can be filled by a single qualified person who has the appropriate certifications required for the roles. (See response below).

2. Can the Superintendent triple hat and cover all 3 roles?

RESPONSE: Yes, as long as the person is qualified and has appropriate certifications required for the roles.

3. Has the request for funding for this project been approved and funds available on station?

RESPONSE: This is internal information.

4. Will the project have to be done in phases?

RESPONSE: Yes. At a minimum, 2 but possibly 3. This will be determined as part of the design phase.

5. Will any temporary systems have to be provided (HVAC, Generators, Power, etc)?

RESPONSE: No.

6. Will the contractor be responsible for removal and disposal of the existing IR and CT equipment?

RESPONSE: Contractor will be responsible for coordination of the installation and removal of the equipment which will become the manufacturer’s property for salvage value. See Scope of Work, Section 2 Scope of Work #3) for details of removal and disposal.

7. Is there an existing overhead door or oversized opening that the contractor can use to remove the equipment?

RESPONSE: There is an oversized sliding door that will be used for ingress and egress to the work zone on the east side of the hospital with access to a service drive.

8. Is all magnetic shielding and radiation shielding already in place?

RESPONSE: No, See Scope of Work, Section 2 Scope of Work #9) for details on shielding.

9. Will additional magnetic shielding and radiation shielding be required in the floors or ceilings?

RESPONSE: Yes, See Scope of Work, Section 2 Scope of Work #9) for details on shielding.

10. Please provide the manufacturer and model numbers for the existing IR and CT equipment?

RESPONSE: The current manufacturer is Philips, for both CT and IR. The model numbers are as follows:

➢ CT model: CT Ingenuity System (728326), REF #728323, and SN 320476.

➢ IR model: IR Allura XPER FD 20 System Module 000444, SN 000607

11. Is there a floor or crawl space below the work area or is it the bottom floor with slab on grade?

RESPONSE: There is a crawl space in the sub-basement beneath the work area.

12. Will the existing structure’s design be able to hold the weight of the new equipment?

RESPONSE: The specific design will require the design-build team’s structural engineer to determine the appropriate load distribution requirements. The structural design of the original building is 100 lbs. PSF. See Scope of Work; Section 6. General Requirements D.

Structural for details.

13. Will the contractor be required to obtain a physicist to design any shielding systems or will the VA provide the design requirement for this?

RESPONSE: The VA Radiation Safety Officer (RSO) will provide direction on shielding requirements.

14. Does the contractor need to include a signed SF1442 in Phase I proposal?

RESPONSE: Reference Solicitation page 11; D. Phase I Evaluation section.

15. The key personnel for construction include Project Engineer. It then states to provide the project engineer’s state of license. A construction project engineer is just a title, they are not actual Engineers with engineering licenses. Please clarify?

RESPONSE: If the project engineer is a part of the AE team as would be expected, they would also be expected to be licensed.
