About this file
This is an exhibit document for the Miami-Dade Aviation Department (MDAD) HVAC Chillers full-service contract opportunity in Florida. The exhibit provides a sample report template that contractors must follow when submitting documentation and performance reports throughout the contract term. The sample report format establishes standardized requirements for how contractors will document their maintenance activities, system inspections, and compliance with Original Equipment Manufacturer (OEM) standards and applicable Federal, State, and Local Laws and Environmental Regulations. The template demonstrates the specific data fields, report structure, and documentation standards that the successful contractor must utilize to ensure consistent and comprehensive tracking of all HVAC system maintenance and auxiliary equipment servicing across the Miami-Dade Aviation Department's facilities.
The sample report exhibit serves as a critical compliance tool that enables MDAD to monitor contractor performance and verify adherence to contract specifications throughout the service period. By establishing this standardized reporting format in advance, the solicitation ensures that all bidders understand the administrative and documentation expectations, allowing for accurate cost estimation and operational planning. The report template reflects MDAD's commitment to maintaining detailed records of all HVAC system preservation activities and ensuring transparent accountability in the full-service contract execution.

Today I checked the condenser water bypass loop for proper flow. I also checked the chemical feed pumps for prime and proper operation. The water chemistry was all within the recommended range of control. Everything was in good condition.
Your condenser water system is properly protected against corrosion and mineral scale deposition.
The CHW system is in need of additional treatment. I will come back and add CST-1308 to the loop to bring the levels more into the appropriate range.
Supplementary biocide was added to the tower in order to aid in the control of microbiological growth.
