About this file
This is a Statement of Work (Exhibit B) for the Colorado Department of Public Health and Environment (CDPHE) Primary Care Office outlining data validation services for the Colorado Health Systems Directory (CHSD). The CHSD aggregates information about licensed clinicians and healthcare sites across Colorado from multiple public and proprietary sources to identify health professional workforce shortages and inform practice incentive allocation. The contractor shall provide CASS-certified address data validation and demographic data validation via application program interface (API), including standardization and correction of address components (street address, city, state, ZIP+4 code, county, census tract, block, and rooftop-level geocoding with six and nine-digit coordinates and latitude/longitude) and demographic data points (sex, date of birth, email address, and education level). The contractor must submit the signed CDPHE Data Use Agreement no later than 30 days after contract execution. Deliverables include accurate address validations and demographic data validations completed via API, with success measured by the number of accurate validations completed.
The Statement of Work does not specify pricing terms, contract duration, or renewal options within the document. The contractor must maintain at least 99% server uptime, adhere to the CDPHE Data Use Agreement (to be provided within seven business days of contract execution), and provide monthly usage monitoring access via API to the CDPHE PCO Database Contractor. The contractor is strictly prohibited from selling, licensing, aggregating, commingling, or otherwise using state data for purposes outside the contracted services, including enriching reference datasets, training machine learning models, or improving artificial intelligence systems. Upon request, the contractor may be required to provide additional CASS-certified standardization and correction on additional data points and validated demographic data. Deliverables should be submitted to tamara.davis@state.co.us.

Exhibit B Statement of Work To Original Contract Number CT XXXX*XXXX These provisions are to be read and interpreted in conjunction with the provisions of the contract specified above, as amended.
I. Entity Name: TBD
II. Project Description The Primary Care Office (PCO) developed and administers the Colorado Health Systems Directory (CHSD), a health information project, which provides comprehensive data on licensed clinicians and health care sites across Colorado. The CHSD aggregates information about clinicians and systems from multiple public and proprietary data sources, matches the information to data in the system, standardizes information, and applies a scoring algorithm to determine current information for clinicians and systems.
The PCO uses the directory to determine where health professional workforce shortages exist in the state and directs practice incentives to those areas of the state determined to have a shortage. To plan for existing and future health professional workforce needs in Colorado and statutory requirements, the CHSD provides needed accurate data about where clinicians practice, how they practice, and how much patient care they provide.
An adequate health professional workforce is a key precondition for access to health care, particularly in low-income and rural communities throughout Colorado. Access to care is a common interest of multiple organizations working in the state on health care delivery, planning, policy and financing. Accurate health professional workforce data needs are a common problem among public health agencies, sister state agencies, governmental, state-chartered, research entities and other stakeholders. The CHSD is a collaborative solution to these problems. The PCO shares directory data with stakeholders, who use the data for a variety of uses including counts of providers in regions of the state to network adequacy.
To assure accurate data provided from the CHSC, data validation via an application program interface (API) that can interface directly with the CHSD is required to validate address and individual demographic data and document the data validation accuracy, with the validated data being returned and populated into the CHSD.
III. Definitions A. API: Application Program Interface B. CASS: Coding Accuracy Support System C. CASS-certified: a program developed by the USPS to test the accuracy of addressing-matching software and improve the accuracy of posting coding, such as ZIP+4 Code, carrier route, and 5-digit ZIP Code coding D. CDPHE: Colorado Department of Public Health and Environment E. CHSD: Colorado Health Systems Directory F. Data validation: checking the accuracy and quality of source data before using, importing, or otherwise processing data G. PCO: Primary Care Office H. Rooftop level geocoding: takes and combines data from multiple sources, such as census and navigation data, and through the use of location algorithms, pinpoints the exact latitude and longitude of a street address on a map
