About this file

This document contains a past performance relevancy questionnaire template for a federal contract opportunity to repair parking lots. The template requests information on up to three of the offeror's most relevant prior contracts, including a description of the work performed, completion dates, contract values, any modifications, subcontractors used, and a point of contact. Relevant work experience must have occurred within the last three years. Additional questions seek the names of any subcontractors or teaming partners proposed for the parking lot repair work, limiting the response to three subcontractors per discipline. The related federal opportunity is solicitation number 36C24222R0102 from the Department of Veterans Affairs for repair of parking lots at a Veterans Health Administration facility. An amendment provided final drawings.
