Ammo Data Cards (ADC) shall be prepared IAW MIL-STD-1168 (current revision) and electronic submission shall be accomplished by accessing the Worldwide Ammunition Data Repository Program (WARP) and shall follow the format required by the worldwide web application identified as WARP or Worldwide Ammunition-data Repository Program at https://mhp.redstone.army.mil.

Input a sample card into WARP thirty (30) days prior to First Article Test (FAT), or production if FAT is not applicable, for review and approval of technical content. When submitting the sample card for review, include the words "SAMPLE CARD ONLY" in the remarks section of the ADC.

Hard copy ADCs shall be generated from WARP and must accompany each production lot.

Card shall be signed and dated by the Government Agency Responsible for Government Acceptance (GARA) usually the DCMA QAR or other Government designee. When ballistic tests at Government proving grounds are required, two (2) hard copy ADCs shall be generated and accompany the lot. Further guidance for this requirement shall be specified/detailed in each order.

*****Every avenue shall be pursued to ensure an accurate ADC is produced with all appropriate documentation included. Any unknown components, older components, off the shelf items and different lot interfix numbers (with the reason for different interfix numbers) can be annotated in the "REMARKS SECTION". This includes any and all information that is unavailable at the time of production.

*****With the permission of the GARA, older lots of ammunition that undergo the renovation process may not contain all required information. this unknown information can clarified in the REMARKS. Any additional special conditions that apply to the lot shall be documented in the REMARKS .

NOTE: Submission shall commence with the first deliverable lot and continue with each subsequent lot. ADCs shall include procurement acquisition numbers for each applicable Request for Deviation, Notice of Revision and Engineering Change Proposal listed on the card.

Submit an email Letter of Transmission (LT) notifying the Product Quality Manager identified below when a card has been entered into WARP.

Block 10 and 12: The Contractor shall report immediately any major accident/incident (including fire) resulting in any one or more of the following: causing one or more fatalities, or one or more disabling injuries; exceeding $10,000 of Government property damage;

affecting planning or production schedules; degrading the safety of equipment under contract, such that personal injury or property damage may be involved; identifying a potential hazard requiring corrective action. The Contractor shall prepare the report in accordance with DI-SAFT-81563 for each incident in accordance with the CDRL.

Block 12 and 13: The Contractor shall prepare a Safety Assessment Report (SAR) of the item IAW Contract Data Requirements List (CDRL) requirements and deliver the SAR to the Government within ninety (90) days prior to testing at a government facility. The SAR should evaluate the risks associated with testing/demonstrating a system at the government facility, identify all safety features of the system and all related hazards associated with testing/demonstrating, and the mitigations (procedures, precautions, etc.) required to minimize risks and safely conduct testing. The SAR shall contain range safety data (e.g.

recommended Surface Danger Zone), results from the contractor’s safety assessments, hazard analyses, and testing IAW DI-SAFT-80102C. MIL-STD-882 (latest revision) can be used for guidance. One SAR shall be submitted for each unique configuration.

Block 12 and 13: The Contractor is responsible for obtaining Final Hazard Classification and an EX number for the MXX from the Department of Transportation. The Contractor shall make certain that the material is tested IAW Part 173, Subpart C, Section 173.58(a) of Title 49 Code of Federal Regulations (CFR) Parts 106-180 to assign proper class and division for all explosives (Division 1.1, 1.2, 1.3, and 1.4 explosives). Registration with the Associate Administrator of Hazardous Materials Safety is required IAW Part 173, Subpart , Section 173.56(b)(1) or 173.56(c) of Title 49 CFR so that markings in accordance with 172, Subpart D, Section 172.301(a) and 172.302(a) are met. Hazard classification documents, to to include the final DOT approved EX#, shall be forwarded to the U.S. Army RDECOM-ARDEC, ATTN: RDAR-EIQ-SF, Picatinny Arsenal, NJ 07806-5000 IAW DI-MISC-80508, Technical Report.

Block 12 and 13: The Contractor may, at his option, utilize the capability of ARDEC to obtain Interim Hazard Classification (IHC) for shipment of R&D quantities of energetic materials/items in support of this contract. If the Contractor elects to use RDECOM-ARDEC for this support, the Contractor will need to provide technical data to RDECOM-ARDEC sixty (60) days prior to shipment of the energetic materials/items. The necessary data is explained in Army Technical Bulletin (TB) 700-2 and DI-SAFT-81299C.

DOT and UN serial number information and packaging method are available in Title 49, Code of Federal Regulations (CFR). The explosive weight for quantity-distance determination is obtained using the guidance of paragraph 15.4 of AMC-R 385-100. If, during the contract, the Contractor needs to obtain or handle alternate explosives for this SOW, then this SOW must be resubmitted to the System Safety Office so that the appropriate safety data can be included in the SOW, if available.

Block 12 and 13: For energetic materials or energetic items made in the US, the Contractor is responsible for obtaining hazard classification from the Department of Transportation prior to any shipment of energetic materials. All US manufacturers shall make certain that the material is tested IAW Part 173, Subpart C, Section 173.58(a) of Title 49, CFR Parts 106-180 to assign proper class and division for all explosives. Registration with the Associate Administrator of Hazardous Materials Safety is required IAW Part 173, Subpart C, Section 173.56(b)(1) or 173.56(c) of Title 49 CFR so that marking in accordance with Part 172, Subpart D, Section 172.301(a) and 172.32(a) are met. Hazard classification documents shall be submitted IAW DI-MISC-80508B.

Block 12 and 13: For energetic materials or energetic items from a foreign country, the foreign company shall provide specific hazard classification. This specific hazard classification information should include, at a minimum, a letter from the Competent Authority issued in accordance with part 2, Chapter 2.0 and Chapter 2.1, paragraph 2.1.2 of United Nations-Transport of Dangerous Goods, ST/SG/AC.10/1 Latest Revision, approves the hazard and a compatibility group assignment, and assigns an appropriate shipping name to the energetic material or energetic item. If available, the test data from running UN Test Series 4(a), 4(b)(ii), 6(a) or 6(b) and 6(c) should be obtained. The information and test data shall be submitted IAW DI-MISC-80508B.

Block 12 and 13: The Contractor shall perform a System Safety Hazard Analysis Report (SSHAR) (DI-SAFT-80101C) IAW MIL-STD-882D. MIL-STD-882D shall be used as a guide to establish severity and probability levels for all hazards that are assessed. The report shall be updated throughout the contract to provide a continuous hazard tracking capability.

The SSHAR shall address both hardware and software safety as an integral part of the overall system safety effort.

Block 12 and 13: The Contractor shall provide records to verify that all materials used for the manufacturing of the munitions are in compliance with the materials, construction and design requirements specified in the TDP or specifications for each lot of cartridges submitted for acceptance, including a certification that Class 1 Ozone Depleting Chemicals (ODCs) are not being used. A Certificate of Compliance shall be prepared and submitted to the US Government representative IAW DI-MISC-81356A. The Contractor shall also advise that this procurement does not include any of the Designated Items identified in the Environmental Protection Agency (EPA's) Comprehensive Procurement Guidelines (CPG).

Parts II and III are not applicable.
