PAST PERFORMANCE QUESTIONNAIRE INSTRUCTIONS
(Updated May, 2018)
Sections 1-3 provide for contract-related descriptive information and identification of the evaluator.
Section 4 lists the major work elements within our Statement of Work (SOW). Please provide your assessment of the “relevant experience” and “performance” associated with our SOW evidenced within the contract for which you are a reference. “Experience” means a contractor who has performed work in the specific SOW Element. “Did not Perform” means that the specific SOW element was not performed under the contract.
Section 5 evaluates the contractor’s technical, schedule, and cost performance and management. (Additional pages may be used for comments if desired). If you cannot answer any questions, please circle “N/R” for Not Rated.
The following definitions are offered for your use in assigning a performance rating for each of the factors in Sections 5 and 6:
Very High
(VH)
The Offeror’s relevant past performance is of exceptional merit;
indicating exemplary performance in a timely, efficient, and economical manner; very minor (if any) problems with no adverse effect on overall performance.
High
(H)
The Offeror’s relevant past performance demonstrates very effective performance that would be fully responsive to contract requirements with contract requirements accomplished in a timely, efficient, and economical manner for the most part with only minor problems with little identifiable effect on overall performance.
Moderate
(M)
The Offeror’s relevant past performance demonstrates effective performance; fully responsive to contract requirements; reportable problems, but with little identifiable effect on overall performance.
Low
(L)
The Offeror’s relevant past performance meets or slightly exceeds minimum acceptable standards; adequate results; reportable problems with identifiable, but not substantial, effects on overall performance.
Very Low
(VL)
The Offeror’s relevant past performance does not meet minimum acceptable standards in one or more areas; remedial action required in one or more areas; problems in one or more areas which, adversely affect overall performance.
N/R Not Rated
Section 6 provides for evaluation of the contractor’s management of cost and award/incentive fee history.

Planning & Scheduling: Correlate Schedules with Technical and approved Budgets. The Contractor shall interface with the technical product, element, or project leads and related resources personnel to correlate project schedules with the current approved budget profiles and plans.
EVM support to management. This includes but not limited to: EVM and IBR management training; Independent EVM assessments and reporting; Evaluation and implementation of EVM tools and interfaces; EVM Peer Reviews; Development and delivery of EVM and IBR training courses; creating and maintaining Center EVM documents and website content; and recommending EVM standards and processes.
Resources & Financial Management. The contractor shall formulate project budgets including labor, travel, and procurements detailed in the Work Breakdown Structure (WBS) structure of each project or program.
Cost Estimation. The contractor will develop project lifecycle cost estimates using NASA-approved commercial cost estimating tools. Working with Flight project scientists and systems engineers to validate technical baseline for cost estimation, advanced, and statistical cost modeling.
Configuration Management (CM). CM is a function which maintains the integrity of the mission or element build. CM is required by each Project office for in-house missions, as well as contracted ones, to be performed to comply with Agency and Center Guidance (Example: GPR
1410.2 or 400-PG-1410.2 or LMS-CP-804). Controlled documentation for hardware and software may encompass documents, drawings, program code, work order authorizations, parts lists, and other build documentation. Each mission is required to have a Configuration Management System (CMS) to keep track of programmatic, technical, and administrative issues and their resolution throughout its life.
Documentation Management. Control, Storage, Dissemination and Tracking of Project Documentation. The Contractor shall develop and update systems for the control, storage, and dissemination of all project documentation and configuration management-controlled documentation; and maintain systems for tracking and retrieving these materials and data.
Financial Services. The contractor shall provide comprehensive support for a portfolio of business systems and processes. Systems supported include, but are not limited to, SAP Core

Financial, Business Objects (BOBJ) and Concur Government Edition (CGE). Collaboration with other organizations to provide support, system updates, and financial process information is required.
Accounting. The contractor shall provide professional accounting consulting support to NASA by analyzing, researching, and resolving complex accounting issues including, but not limited to, monthly and quarterly accounting activities, the annual financial statement audit, and advising on accounting standards and reporting requirements.
Travel System Processing. The contractor shall account for, prepare, and collect documentation for total travel (planned vs. actual); and coordinate travel arrangements in accordance with requirements for both domestic and OCONUS travel. Ensure funding is available and funding information is correct and accurate. Respond to inquiries from, and initiate inquiries to, travelers and other authorized personnel to secure additional or corrected information necessary for the processing of documents within the travel system.
Project Operations Support. The contractor shall provide operations support to programs, projects, and offices to include coordination of support activities, facility operations, Government property management, collaborations, technical documentation, project lifecycle reviews, and spaceflight hardware logistics.
