CONTRACTOR PERFORMANCE STATUS REPORT
This monthly Contractor Performance Status Report (CPSR) is designed to provide contractor award data on this RHCO-E contract/task orders. The CPSR consists of five (5) tabs labeled: Instructions, Task Order, Worker, Assignment & Government Actions. The CPSR must be submitted no later than the 10th calendar day following the end of the month. Description of each tab is outlined below:
TAB 1: Instructions – Tab fill instructions
TAB 2: Task Order - This tab is designed to be imported into a Government surveillance application. It provides the task orders that were awarded to the Contractor during the reporting period.
TAB 3: Worker - This tab is designed to be imported into a Government surveillance application. It provides the list of HCW who are employed by the Contractor and will be performing on this contract/task orders.
TAB 4: Assignment - This tab is designed to be imported into a Government surveillance application. It provides the assignment of a HCW to a specific task order and position at a specific level of effort or full time equivalent (FTE).
Each HCW assignment, either initial or replacement, must be reported on this tab. A replacement assignment can only occur after the assignment of the replaced HCW has ended (i.e., John, whose level of effort is 1 FTE, can only be listed as a replacement for Joe, whose level of effort is also 1 FTE, when Joe's assignment has ended. This requires an entry for Joe with an end date of his assignment). The total number of active assigned FTEs on a labor category cannot exceed the required number of FTEs for the labor category.
TAB 5: Government Actions - This tab is designed for the Contractor to report Government action during the month. The columns are available for reporting on each Government action.
