About this file
This document is an Evaluation Factors addendum that outlines the criteria for evaluating proposals submitted in response to solicitation W912PQ26QA031, a Firm Fixed Price Indefinite Delivery/Indefinite Quantity (IDIQ) contract for Periodic Health Assessments (PHA) services throughout New York State for the New York Army National Guard. The evaluation will employ a two-step process: price will be evaluated first, and the apparent lowest-price proposal will be assessed for acceptability under technical factors before past performance is reviewed. The evaluation employs five primary factors: (1) Technical Approach and Understanding, which assesses the offeror's comprehension of requirements and quality of their execution plan for PHA events, staffing model capable of processing over 300 soldiers per 8-hour period, and detailed training plan for all staff; (2) Key Personnel Qualifications and Experience, evaluating the credentials of the proposed Event Coordinator, medical providers (MD, DO, NP, PA) with preference for military or related experience, and technical proficiency with required IT systems including Microsoft Office and military systems such as MEDPROS, MEDCHART, and MODS; (3) Quality Control, requiring submission of a preliminary Quality Control Plan detailing deficiency identification, prevention, and correction; (4) Insurance and Indemnification, demonstrating ability to meet Commercial General Liability, Professional Liability/Medical Malpractice, and Cyber Liability requirements; and (5) Past Performance, limited to two pages and evaluated as either Relevant or Neutral based on experience within the past three years performing dental examination services with government agencies.
Awards will be made without discussions to the lowest-priced acceptable offeror; however, the Government reserves the right to conduct discussions or award to other than the lowest-priced offeror if determined to result in best value. This is a total small business set-aside with a NAICS code of 621112 and size standard of $13,500,000.00, issued by the USP&FO for New York, Purchasing and Contracting Division. Proposals must not exceed 25 pages with font no smaller than 10 points and must include the pricing sheet. The quotes deadline is August 27, 2026, at 9:00 a.m. ET, with all questions to be submitted by August 18, 2026, at 3:00 p.m. ET to the specified contracting officers.

Addendum to 52.212-5
Evaluation Factors:
Price plus other factors being considered:
Factors 1-5: The agency will first evaluate price, and then evaluate the apparent lowest-price proposal for acceptability.
1. Technical Approach and Understanding:
This factor assesses the Offeror's understanding of the requirements and the quality of their proposed plan to meet them.
· Event execution plan: The contractor's proposed methodology for executing PHA events, as outlined in PWS section 5.3, including:
· Soldier check-in and check-out procedures.
· Patient flow management and soldier tracking.
· Documentation and record-keeping processes.
· Plan for post-event reporting.
· Staffing Plan: The adequacy and efficiency of the proposed staffing model to meet the throughput requirements mentioned in PWS section 3.7 (e.g., processing over 300 soldiers per 8-hour period)
· Training Plan: The contractor is required to provide a detailed training plan with their proposal (PWS Section 5.5). The quality and thoroughness of this plan for all staff, especially for Health Care Professionals (HCPs) on military-specific systems, is a critical evaluation point.
2. Key Personnel Qualifications and Experience The PWS emphasizes the importance of qualified personnel. Offeror’s will be evaluated on the experience and credentials of their proposed key staff.
· Event Coordinator: The experience and qualifications of the proposed Event Coordinator in managing similar events (PWS Section 3.1)
· PHA Providers (MD, DO, NP, PA): The qualifications, licensure, and experience of the proposed medical providers (PWS Section 3.2, 3.5). The PWS states a preference for providers with prior military or related experience (PWS Section 1.4.13), which can be an evaluation criterion.
· Technical Skills: The proficiency of proposed personnel with required IT systems, including Microsoft Office and, preferably, military systems like MEDPROS, MEDCHART, and MODS (PWS Section 1.4.12).
3. Quality Control The contractor’s ability to ensure high-quality service is a significant factor.
· Quality Control Plan (QCP): The PWS requires offeror’s to submit a preliminary QCP with their proposal (PWS Section 1.4.1). This plan should detail how the contractor will identify, prevent, and correct service deficiencies and ensure compliance with all PWS requirements. The thoroughness and effectiveness of the proposed QCP would be a major evaluation factor.
4. Insurance and Indemnification The offeror must demonstrate the ability to meet the insurance requirements outlined in section 3.8, including Commercial General Liability, Professional Liability/Medical Malpractice, and Cyber Liability insurance.
5. Past Performance Evaluating an offeror's history of successfully performing similar work provides an indication of their ability to perform successfully on this contract. This would involve checking references for contracts of a similar scope and complexity. - This will be evaluated as Relevant, Neutral and all experience provided shall have been in the past 3 years. Contractor must provide no more than 2 pages detailing their experience. --- If No past performance - Contractor shall say "they have no past performance" in this area and will received a "Neutral" rating. To be considered "Relevant" the contractor experience shall show they have the following: Experience: (Dental Examination Experience with government agencies- 1 Number of years doing 2 or Similar. If similar you must show how it relates to this requirement) Note, this is regarding the contractor themselves NOT the employees who will be working on contract if for services.
The entire quote submitted shall not exceed 25 pages and have a font smaller than 10pt. It is anticipated that award will be made without discussions. However, the Government reserves the right to conduct discussions or negotiations if necessary.
The agency will first evaluate price, and then evaluate the apparent lowest-price proposal for acceptability under the factors 1-4 listed above and if acceptable will review factor 5- past performance. If past performance is considered "Relevant" award will be made. If past performance is "Neutral" the next lowest price acceptable offer will be evaluated, continuing this process as necessary. The Government requires a contractor with more experience; however, will not pay substantially more for it. The government reserves the right to award to other than the lowest-priced offeror or the offeror with the highest rating if determined to do so would result in the best value to the Government. This is being awarded using RFO FAR 12.
