About this file

This Addendum document contains Summary of Benefits and Coverage (SBC) documents for three health insurance plan options offered by Travis County Healthcare District–Central Health, an Aetna-administered EPO plan with coverage period January 1–December 31, 2026. The three plan options detailed are: (1) Aetna Open Access Aetna Select HDHP, (2) Aetna Open Access Aetna Select Texas Med Neighborhood, and (3) Seton Open Access Aetna Select ACO. Coverage is available for Individual + Family tiers across all three plans. Central Health is requesting proposals for hospital indemnity insurance coverage for approximately 2,300 active employees and their dependents, with proposals due July 13, 2026 at 2:00 p.m. Central Time and coverage effective January 1, 2027. The contract term is one year with four optional one-year renewal periods at Central Health's discretion.

The three plan options present varying cost-sharing structures. The HDHP plan features an individual deductible of $3,300 and family deductible of $6,600 with an individual out-of-pocket limit of $4,000 and family limit of $8,000, and does not meet Minimum Value Standards. The Texas Med Neighborhood plan and Seton Health ACO plan both offer lower individual deductibles of $1,500 and $1,000 respectively, with family deductibles of $2,500 and $2,000, both maintaining individual and family out-of-pocket limits of $4,000 and $8,000 respectively, and both meet Minimum Value Standards. All three plans cover preventive care before deductible is met, offer no referrals required for specialist visits, and provide 20% coinsurance for most services after deductible is satisfied. Copayments range from $15–$75 for prescription drugs and office visits depending on plan tier and service type. Pricing must be submitted net of commissions with all fees itemized, and carriers must accept 60-day payment terms and waive "actively at work" provisions.

Summary of Benefits and Coverage: What this Plan Covers & What You Pay for Covered Services TRAVIS COUNTY HEALTHCARE DISTRICT - CENTRAL HEALTH : Aetna

Open Access® Aetna SelectSM - HDHP

Coverage Period: 01/01/2026-12/31/2026

Coverage for: Individual + Family | Plan Type: EPO

The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately. This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, www.HealthReformPlanSBC.com or by calling 1- 888-982-3862. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. You can view the Glossary at https://www.healthcare.gov/sbc-glossary/ or call 1-888-982-3862 to request a copy.

Important Questions Answers Why This Matters:

What is the overall deductible? In-Network: Individual $3,300 / Family $6,600.

Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.

Are there services covered before you meet your deductible?

Yes. In-network preventive care is covered before you meet your deductible.

This plan covers some items and services even if you haven't yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible.

See a list of covered preventive services at https://www.healthcare.gov/coverage/preventive-care-benefits/

Are there other deductibles for specific services? No. You don’t have to meet deductibles for specific services.

What is the out-of-pocket limit for this plan? In-Network: Individual $4,000 / Family $8,000.

The out–of–pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out–of–pocket limits until the overall family out–of–pocket limit has been met.

What is not included in the out-of-pocket limit?

Premiums, balance-billing charges & health care this plan doesn't cover. Even though you pay these expenses, they don’t count toward the out–of–pocket limit.

Will you pay less if you use a network provider?

Yes. See www.aetna.com/docfind or call 1-888- 982-3862 for a list of in-network providers.

This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider's charge and what your plan pays (balance billing). Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.

Do you need a referral to see a specialist? No. You can see the specialist you choose without a referral.

TRAVIS COUNTY HEALTHCARE DISTRICT - CENTRAL HEALTH : Seton

Open Access® Aetna SelectSM - ACO

Seton Health In-Network: Individual $1,000 / Family $2,000.

Yes. In-network office visits, prescription drugs & preventive care are covered before you meet your deductible.

Seton Health In-Network: Individual $4,000 / Family $8,000.

Yes. See www.aetna.com/docfind or call 1-888- 982-3862 for a list of Seton Health In-Network providers.
