About this file
This is a Texas Family Code Schedule certification document required for Solicitation No. 601330000045576 issued by the State of Texas. The schedule serves as an eligibility verification form that respondents must complete, sign, and return as part of their proposal submission. The document establishes compliance with Texas Family Code Section 231.006 regarding child support obligations, requiring respondents to certify that the individual or business entity submitting the response is eligible to receive state contract payments. Respondents must acknowledge that failure to provide accurate information may result in contract termination and payment withholding. The schedule must be returned with the response, and failure to sign and return the completed document may render the response non-responsive to the solicitation.
Any business entity submitting a response must provide the names and Social Security numbers of all persons or entities holding at least twenty-five percent ownership interest. The Federal Privacy Act Notice included in the schedule discloses that Social Security numbers are required under Texas Family Code Sections 231.006(c) and 231.302(c)(2) for identification of individuals who may owe child support obligations. The SSN information will be kept confidential to the fullest extent permitted under Section 231.302(e) of the Texas Family Code. Respondents who do not meet the ownership criteria may check a box indicating that the schedule is not applicable to their situation.
