About this file

This is a Security and Privacy Inquiry (SPI) Form required by the Texas Workforce Commission (TWC) for vendors seeking access to TWC confidential information in connection with Employee Verification Services. The form establishes mandatory security and privacy compliance requirements that vendors must meet to handle sensitive TWC data. Vendors must provide comprehensive documentation including business information, designated security and privacy officials, details on employee and subcontractor personnel accessing confidential information, authorization boundary diagrams, and data flow diagrams depicting how information is protected across production, test, development, and alternate sites. The form requires vendors to demonstrate compliance with multiple security domains including access control, incident response, media protection, physical and environmental protection, system communications protection, and supply chain risk management.

The form mandates that vendors implement written privacy and security policies addressing transparency, individual participation, purpose specification, data minimization, use limitation, data quality and integrity, and accountability with auditing capabilities. Vendors must confirm they do not process, transmit, or store TWC data outside the United States and that encryption keys are held exclusively within U.S. borders. Additional requirements include procedures for terminating access upon employee or subcontractor separation, data sanitization protocols upon contract termination, unique user identifiers and complex password policies, FedRAMP and/or TX-RAMP certification status for any cloud services utilized, and appropriate security safeguards for personally identifiable information. The form specifies data protection requirements for data at rest, data in use, and data in motion, requiring role-based access controls, mandatory user training, and encryption via VPN tunnels, HTTPS, or SSL/TLS protocols.
