About this file
This is Addendum No. 4 to Request for Offers (RFO) No. HHS0014612 issued by Texas Health and Human Services for Take Charge Texas Support Services. The addendum makes three significant modifications to the original RFO: 1) It replaces Section 2.3.3 regarding Disaster Recovery Test requirements, specifying that the contractor must participate in one business continuity preparedness exercise per State Fiscal Year without additional compensation, with 30 days advance notice from DSHS; 2) It adds Section 2.4.7 Place of Performance, requiring all work to be performed remotely within the contiguous United States; and 3) It amends Section 3.7 by adding Artifact B – TCT Service Design and Architecture_Addendum 3 to the procurement library.
The document includes the Texas Health and Human Services logo and is formatted as a standard government contract amendment document. The addendum makes no modifications to pricing terms, set-asides, or funding requirements. The changes focus primarily on operational requirements regarding disaster recovery testing protocols and establishing geographical limitations for where contract work can be performed.
