ASBESTOS REINSPECTION

GENERAL SERVICES ADMINISTRATION

FEDERAL OFFICE BUILDING, (WA0036ZZ)

SEATTLE, WA

BACKGROUND

EHS International, Inc. (EHSI), a hazardous materials consulting firm, under contract to the General Services Administration (GSA) was tasked by GSA to conduct a hazardous materials reinspection and confirmation sampling in accessible areas at the Federal Office Building, located at 909 First Avenue, Seattle, Washington.

EXECUTIVE SUMMARY OF FINDINGS

The following suspect materials were assumed to be asbestos containing or sampled and identified as either asbestos-containing materials (ACM) or non-ACM in the subject areas of the building:

Asbestos-Containing Materials (ACM) Sampled Non-Asbestos Materials Pipe thermal system insulation (TSI) Gypsum wallboard (GWB) ACM dark brown mastic associated with non-ACM 12” acoustic ceiling tile (ACT) with dot matrix pattern

Joint Compound (JC)

Roofing 2’x4’ suspended acoustic ceiling tile (SACT) with random hole pattern

ACM wire insulation Floor pipe penetration packing/sealant Fire doors Brown HVAC seam sealant

Assumed ACM Gray HVAC seam sealant Vinyl asbestos tile (VAT) and ACM mastic under carpet in elevator interiors Window glazing

HVAC vibration cloth Blue spray applied fireproofing Brick and mortar Tan glue dots associated with marble cove base/splashguard walls 4” black vinyl cove base with dark brown mastic Caulking associated with marble cove base Yellow carpet mastic Plaster Gray stair tread with black mastic 6” beige vinyl cove base with white mastic Gray sheet vinyl (SV) with tan mastic

DAMAGED MATERIAL

No damaged asbestos-containing materials were noted in the building.

REINSPECTION

EHSI accomplished an asbestos materials reinspection of the subject building on March 13, 2007. The intent of the reinspection is to specifically identify, quantify, determine the current condition, assign homogenous sample area numbers (HSA#), and develop an engineer’s cost estimate for removal/disposal of ACM.

EHSI was tasked to use applicable Federal, State, and local guidelines in the accomplishment of its reinspection. EHSI was further tasked to provide a written report for the areas reinspected including identification of all suspect materials and a determination of whether suspect material was ACM.

A spreadsheet listing all of the reinspection data is included in Appendix A.

REINSPECTION FINDINGS

Asbestos-Containing Materials

EHSI reviewed the GSA files for previous asbestos inspections kept on file in the Jackson Federal Building in Seattle, WA. A May 2005 report by PBS and a July 2005 report by EHSI did not list any building materials as sampled and confirmed asbestos-containing materials. 1987 and 1988 surveys by Industrial Health Inc. listed pipe TSI and fire doors as sampled and confirmed asbestos-containing materials. A 1990 report from Centrac listed roofing as a sampled and confirmed asbestos-containing material. Previous asbestos abatement design drawings from Centrac list various concealed areas of ACM wire insulation and ACM pipe TSI as being left in place. ACM dark brown mastic associated with non-ACM 12” ACT and roofing material are also confirmed asbestos-containing material. Additional survey samples taken previously confirmed that the following materials were not ACM: joint compound, plaster, 2’x 4’ suspended acoustical ceiling tile (SACT) and sheet vinyl flooring (SV) flooring in the South stairwell.

This building, which was reportedly built in 1930, has undergone past renovation activities:

All accessible asbestos containing materials were reported to have been abated from this building. However, ACM may be concealed. in wall and ceiling cavities, including thermal pipe insulation (TSI) and electrical wire insulation, and in fire door cores.

In 2005 repairs were made for a roof leak which impacted the 8th and 9th floors.

The elevators and the 9th floor were undergoing renovation at the time of this survey.

Sixteen (16) bulk samples of suspect ACM were collected as part of this reinspection.

Summaries of asbestos sampling information including sample number, sample location, material description, and analytical results are included in Table 1 below.

ACM identified in this survey should not be disturbed unless handled by personnel who are properly trained and certified in asbestos work. Any survey, regardless of how extensive, can miss concealed materials. Demolition and/or renovation activities by contractors may expose concealed suspect ACM. Contingency plans should include stopping work on identification of concealed suspect ACM, evacuation of the area, and sampling by a certified AHERA inspector. Concealed suspect material may include, but is not limited to:

non-fiberglass pipe insulation; spray-applied coatings; cementitious board; asphalt or paper vapor barriers; and mastics.

LIMITATIONS

During reinspection, not all rooms or areas were accessible due to tenant restrictions.

The entire 9th floor and tower were also inaccessible due to ongoing construction activity which completely blocked access to the 9th floor. This reinspection was limited to the chemicals and materials identified herein. No effort was made to identify hazardous materials in soil, water, or air, other than those listed herein. Limited destructive inspection (e.g., cutting holes in floors, walls, and ceilings) of the building materials to identify, sample, quantify and determine the condition of concealed suspect building materials was not accomplished for this reinspection. Any reinspection, regardless of how extensive, can miss concealed materials. Contractors should be aware of the potential that demolition activities may expose concealed suspect ACM and should have preplanned contingencies for handling these materials when discovered during demolition work. Any concealed, suspect ACM material that was not sampled or that was assumed to be ACM by this reinspection report when encountered must be assumed to be ACM and treated as such until sampled by a certified AHERA Building Inspector and analyzed by a certified laboratory.

STANDARD OF CARE

The recommendations and conclusions contained in this report represent the professional opinions of EHSI. These opinions are derived in accordance with Federal, State and local environmental and health and safety laws and regulations. SAMPLING METHODOLOGY

Asbestos-Containing Materials

The EHSI field inspectors were an asbestos Building Inspector, certified under the requirements of the United States Environmental Protection Agency (EPA) Asbestos Hazard Emergency Response Act (AHERA) regulation 40 CFR 763, Subpart E. Sampling was consistent with the requirements of AHERA, as well as meeting the requirements of State and local regulatory agencies. Samples were collected and analytical data obtained for suspect ACM identified in the facility. The number of bulk samples collected and their locations were based on the AHERA regulation and the guidelines provided by the EPA Document 560/5-85-030a, October 1985, Asbestos in Buildings: Simplified Sampling Scheme For Friable Surfacing Materials. Once collected, bulk samples were sealed to eliminate the possibility of contamination. “Chain-of-custody” tracking was followed to maintain sample integrity during handling and data reporting at EHSI and the analytical laboratory. As specified in 40 CFR Chapter 1 (1-1-87 edition) Part 763, Subpart F, Appendix A, each sample was analyzed using polarized light microscopy (PLM) / dispersion staining techniques, in accordance with U.S. EPA Method 600/R-93/116.

Samples were analyzed for asbestos content by NVL Laboratories, Inc. (NVL), Seattle, Washington, a National Voluntary Laboratory Accreditation Program (NVLAP) accredited laboratory. Only materials containing more than 1% total asbestos were classified as “asbestos-containing” based on EPA, State, and local regulations.

Samples in the report labeled as #QA are Quality Assurance (QA) samples. Split samples were taken from the same location and they were sent to separate laboratories.

QA samples were submitted to, Northern Laboratory and Consulting Services, Inc.

(NLCS), Burien, Washington, another NVLAP laboratory. Copies of both laboratories’ NVLAP certifications are included in Appendix E.

Subject: Seattle Federal Office Building Exterior Window Caulk Samples for Asbestos and PCBs

The exterior window caulk and sealant associated with the windows at the Seattle Federal Office Building were sampled for the presence of asbestos and/or PCBs. Only the exterior portion of the windows were sampled. Of the exterior sealants and glazing, three different types of material were sampled. These included the outer most grey sealant, and thin inner bead of sealant adjacent to the window frame, and a cementitious glazing compound. The results of the testing indicated the presence of asbestos in the outer most grey sealant and the cementitious glazing.

Samples were collected from the following locations:

Asbestos Results

The asbestos samples were placed in plastic bags and delivered to the RGA Environmental Laboratory in Seattle for PLM analysis. The PCB samples were placed in 4oz glass jars and delivered to the Test America Laboratory in Fife. The samples were analyzed by EPA Method 8082. The laboratory reports are attached.

The asbestos results indicated the presence of 3% chrysotile for the outer sealant on each levels except for the 9th floor. Though the results did not indicate the presence of asbestos in this sample, in accordance with EPA protocols, all outer sealant should be considered as asbestos containing.

A single glazing sample indicated the presence of 3% chrysotile asbestos. Even though all other samples did not indicate the presence of asbestos, they must be treated as asbestos containing, as the glazing compound appears to be similar in all cases. The consideration of all glazing as asbestos is consistent with EPA protocols.

All asbestos containing sealants and glazing must be handled in accordance with EPA and OSHA asbestos regulations. Workers disturbing the sealant and caulking are required to be trained, follow appropriate work practices, and dispose of the material in line with OSHA 29 CFR 1926.1101, WAC 296-62, and EPA FOB40 CFR 61.

PCB Results

The PCB results did not exceed the EPA limit of 50ppm on any of the samples. One sample, FOB-2NGR, did indicate the presence of 0.93 ppm of Aroclor 1254. As no sample was above the EPA limit to be considered a PCB contaminated material, no special handling or waste considerations are required.

Text11:Dear Bob Bliss, Enclosed please find test results for the bulk samples submitted to our laboratory for analysis.

Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy (PLM) with dispersion staining in accordance with U.S. EPA method 600/R- 93/116, “Method for Determination of Asbestos in Bulk Building Materials.”

For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled L-1 for layer one, L-2 for layer two, etc.). For multi-layered samples, NESHAP requires that layers must be analyzed and reported separately. The asbestos concentration in a sample is determined by visual estimation.

For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAP, 40 CFR- Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. If you would like us to further refine the concentration estimates of asbestos in these samples using point counting, please let us know.

This report is considered highly confidential and will not be released without your approval. Samples are archived for thirty (30) days following analysis. Samples that are not retrieved by the client are discarded after thirty (30) days.

This report relates only to the items tested. If samples are not collected by RGA Environmental personnel, accuracy of the results is limited by the methodology and expertise of the sample collector.

Analyses are cross-checked with other laboratories for quality assurance purposes. This report shall not be reproduced except in full, without written approval of RGA Environmental. It shall not be used to claim product endorsement by NVLAP or any other agency of the U.S. Government.

It has been a pleasure to be of service to you. Please call if there is anything further we can assist you with.

Sincerely, Laboratory Director

Receipt

The samples were received on 12/19/2013 1:20 PM; the samples arrived in good condition, properly preserved and, where required, on ice. The temperature of the cooler at receipt was 17.9º C.

Except:

The following samples FOB-1EG (580-41715-8), FOB-1EGL (580-41715-15), FOB-1EGr (580-41715-7), FOB-1NGr (580-41715-4), FOB-1SG (580-41715-5), FOB-1SGr (580-41715-6), FOB-2NG (580-41715-9), FOB-2NGr (580-41715-10), FOB-2SG (580-41715-1), FOB-2SGr (580-41715-2), FOB-9NG (580-41715-12), FOB-9NGr (580-41715-11), FOB-9SG (580-41715-13), FOB-9SGr (580-41715-14), FOB-WG (580-41715-3) were received at the laboratory outside the required temperature criteria. Samples received without ice, cooler, and outside of required temperature at 17.9°C.

GC Semi VOA - Method 8082

In analytical batch 151410 surrogate recovery for the CCV associated with the following samples (CCV 580-151410/53), FOB-1EG

(580-41715-8), FOB-1EGL (580-41715-15), FOB-2NG (580-41715-9), FOB-9NG (580-41715-12), FOB-9NGr (580-41715-11), FOB-9SG

(580-41715-13), FOB-9SGr (580-41715-14) was outside the upper control limit for DCB Decachlorobiphenyl. The samples did not contain any target analytes; therefore, re-extraction and/or re-analysis was not performed.

In analytical batch 151410 the laboratory control sample (LCS) and / or laboratory control sample duplicate (LCSD) for preparation batch

151423 recovered outside control limits for PCB-1260. These analytes were biased high in the LCS and were not detected in the associated samples; therefore, the data have been qualified "*" and reported. The affected samples are: (LCS 580-151423/2-A), (LCSD

580-151423/3-A), FOB-1EG (580-41715-8), FOB-1EGL (580-41715-15), FOB-1EGr (580-41715-7), FOB-1NGr (580-41715-4), FOB-1SG

(580-41715-5), FOB-1SGr (580-41715-6), FOB-2NG (580-41715-9), FOB-2NGr (580-41715-10), FOB-2SG (580-41715-1), FOB-2SGr

(580-41715-2), FOB-9NG (580-41715-12), FOB-9NGr (580-41715-11), FOB-9SG (580-41715-13), FOB-9SGr (580-41715-14), FOB-WG

(580-41715-3).

The continuing calibration verification (CCV) associated with analytical batch 151410 recovered above the upper control limit for

PCB-1260. The closing CCV(53) also failed high for DCB surrogate. The client samples associated with this CCV were non-detects for the affected analytes; therefore, the data have been qualified "^" and reported. The following samples are impacted: (CCB

580-151410/42), (CCV 580-151410/30), (CCV 580-151410/41), (CCV 580-151410/53), (LCS 580-151423/2-A), (LCSD 580-151423/3-A), (MB 580-151423/1-A), FOB-1EG (580-41715-8), FOB-1EGL (580-41715-15), FOB-1EGr (580-41715-7), FOB-1NGr (580-41715-4), FOB-1SG (580-41715-5), FOB-1SGr (580-41715-6), FOB-2NG (580-41715-9), FOB-2NGr (580-41715-10), FOB-2SG (580-41715-1), FOB-2SGr (580-41715-2), FOB-9NG (580-41715-12), FOB-9NGr (580-41715-11), FOB-9SG (580-41715-13), FOB-9SGr (580-41715-14), FOB-WG (580-41715-3).
