BACKGROUND OF COMPANY
Pestmaster Services began servicing pest control needs in April 1979 and has now grown into a national company with 36 franchises across the nation. Our mission is to provide leading edge technologies in the control of insects, rodents, birds, weeds, and other pests.
Pest control services include those to control ants, mice, flies, moths, cockroaches, scorpions, snakes, bed bugs, and a myriad of other pests. We also offer control of termites and other wood-destroying organisms.
STATEMENT OF QUALIFICATIONS
At Pestmaster Services, we are careful and consistent about all parts of our mission. One of the first key points is to provide quality staff and we focus on hiring experienced and knowledgable technicians. When a person applies for employment, we perform a full background screening process, including criminal history, driving record, and reference.
We have a “No Tolerance” policy toward Alcohol and Drug Use which is fully enforced. Only persons with the character and qualifications to meet our standards are hired.Pestmaster will only use experienced technicians and backup technicians for a job of this magnitude and importance. Our technicians will be fully licensed to perform all required pest control tasks and will have experience and training all industry-standard IPM protocols.
If Pestmaster is unable to perform services or have services performed as required, Pestmaster will immediately notify the COR or designee and provide a justification for non-performance and institute the contingency and/or back-up plan as quickly as possible.
Should our lead technician not be available or go on vacation, his work will be performed by our backup technician. Our backup technician will spend as much time as needed for training on the facilities to be able to step in as needed. Mr. Van
Diepen is also trained as a backup technician to further ensure seamless servicing in the unlikely event our other technicians are not available.
Pestmaster has additional local technicians available to handle overflow or in the unusual event that our lead technician is not available. These technicians will be fully licensed and will work under the direction of our project manager.
All Pestmaster Services technicans are licensed and trained prior to entering the field. Our IPM training program has been implemented for over 30 years and we pride our technicans in “green” pest management. Dr. Stuart Mitchell and Jerry
Hatch are our consulting entomologists and have assisted in the development and redevelopment of Pestmaster Services training program for over 15 years.

State and Federal laws regarding methods of application, posting of notices (if required) prior to service, and the closing of any areas for a period of time after service, as well as any other applicable State or Federal regulations.

We have several methods for communicating these emergencies: Via telephone during regular business hours to the Tucson office at 520-373-3261 or via email to contracts@pestmaster.com and kcartwright@pestmaster.com . Should an emergency occur after hours, we request that an initial email be sent to contracts@pestmaster.com and kcartwright@pestmaster.com, followed up by a phone call to Kat Cartwright at 520-373-
3261.
We want to ensure that you are receiving the greenest and most prompt services possible. We are requesting that these procedures be followed should an emergency situation arise. Please be sure that there is a formal understanding as to what constitutes an emergency at these locations.

Surveillance Methods & Frequency: Inspections will be performed at least monthly, during which Pestmaster technicians will perform each of the following tasks:
1. The Pest Sighting Log will be reviewed.
2. All Pest Monitor Stations will be physically inspected.
3. A visual inspection of the location will be made to identify potential sources of sustenance and harborage.
Control Methods: Sanitation will be the primary method of control. Recommendations for sanitation will be made after every inspection. It is the responsibility of the staff to perform all sanitation procedures. Removal of all food, water, and harborage sources are key components of sanitation and are necessary to gain and maintain effective control of pest populations.
One of the new technologies that Pestmaster is utilizing is the Laser Thermometer. This mini infrared thermometer detects temperatures and is able to identify if conditions are conducive for cockroaches and other crawling insects to thrive. The ideal temperature for insects is above 60 degrees Fahrenheit on surfaces, so any application of preventative pesticide is deemed unnecessary in these areas (this is an example of true IPM).
Physical removal will be accomplished by utilizing a vacuum. Vacuuming should be utilized prior to sealing any openings. The use of gel baits, such as Advion or Maxforce, and bait stations will be used only after the site has been properly sanitized and physical removal procedures have been completed.
The use of liquid insecticides and Insect Growth Regulators will be used only in the event of severe and persistent infestation. When such methods are required, applications will be made using crack and crevice and spot treatment methodologies only. Care will be taken not to contaminate any working surfaces.
ANTS
Surveillance Methods & Frequency: Inspections will be performed at least monthly during which Pestmaster technicians
3. A visual inspection of the location will be made to identify potential sources of sustenance and entry.
Control Methods: Exclusion will be the primary method of control. Once a point of entry has been identified it will be sealed using caulk, concrete patch, or other sealant capable of preventing access to ants.
Recommendations for sanitation will be made after every inspection. It is the responsibility of the staff to perform all sanitation procedures. Removal of all food, water, and harborage sources are key components of sanitation and are necessary to maintain effective control of pest populations.

Physical removal of ants will be accomplished by utilizing a vacuum. The use of gel baits, such as Advion, and bait stations will be used only if exclusion, sanitation, and physical removal procedures have proven ineffective. The use of liquid insecticides will be used only in the event of severe and persistent infestation. When such methods are required, applications will be made using crack & crevice and spot treatment methodologies only. Care will be taken not to contaminate any working surfaces.
SPIDERS
Surveillance Methods & Frequency: Inspections will be performed at least monthly during which Pestmaster technicians
3. A visual inspection of the location will be made to identify the presence of spiders.

BEE, WASP, HORNET, AND STINGING ARTHROPOD CONTROL
Integrated Pest Management techniques may be used in some situations by removing nests or improving sanitation, which will remove food sources. In other cases, the key to effective control is with a product such as Wasp Freeze, which will provide a quick „knock down‟ of the infestation. We may use traps which have pheromones inside. These swarm traps will be inspected on a regular basis. When bees are swarming, they are looking for a place to nest and are attracted to these swarm nests. Evidence of swarming activity will then initiate control measures.
As part of our regular inspections on all areas, we will identify nests that are being constructed and will remove those as part of our IPM services. Stinging arthropod control will be an important area for Pestmaster Services to provide a proactive control strategy. Traps and nest removal are the methods we use for control of these and other types of stinging arthropods. Pestmaster Services has discovered that a simple soap and water solution effectively kills bees.
STORED PRODUCT PEST CONTROL
There are a variety of stored product pests from beetles to moths, including saw-tooth grain beetle, red/confused flour beetles, trogoderma, beetles, and grain moths. Again, identification is crucial in choosing the best course of action. There are many pheromones that are specific to the type of pest to be controlled, and we will use these to catch and identify the pest. Pestmaster will again employ IPM strategies and approaches to control the pests. This may include various non-pesticide approaches, as well as green strategies that are effective for stored product pests.
RODENTS
Surveillance Methods & Frequency: Inspections will be performed at least monthly during which Pestmaster technicians
3. A visual inspection of the location will be made to identify damage, droppings, and rub marks caused by rodents.
Control Methods: Exclusion will be the primary method of control. Any openings ¼ inch or larger found during routine inspections will be sealed to prevent rodent entry. Recommendations for sanitation will be made after every inspection.
It is the responsibility of the staff to perform all sanitation procedures. Removal of all food, water, and harborage sources are key components of sanitation and are necessary to maintain effective control of pest populations.
Trapping methods, such as glue boards, live traps, and snap traps, will be used if a rodent infestation is identified during routine inspections. Should an infestation prove to be severe enough or trapping procedures prove to be insufficient, the use of approved rodenticides may be authorized; provided that such rodenticides are secured in locked bait boxes.

FLIES AND MOTHS
Control of flying insects includes the house-fly, filth fly, moths, etc.
Surveillance Methods & Frequency: Inspections will be performed at least monthly during which Pestmaster technicians
2. All installed Vector Fly Lights will be physically inspected.
One method of control is to use discrete placement of our fly lights throughout facilities. The use of liquid insecticides will be used only in the event of a severe and persistent infestation. When such methods are required, applications will be made using crack & crevice and spot treatment methodologies only. Care will be taken not to contaminate any working surfaces.
TERMITE AND WOOD DESTROYING ORGANISMS
Surveillance Methods & Frequency: Inspections will be performed at least monthly during which Pestmaster technicians
3. A visual inspection of the location will be made to identify type of termite and extent of infestation.
If, upon inspection, it is determined that termites are present, the technician will treat for such pests in a manner that is in accordance with the type of termite and the extent of the infestation, and all such treatments shall be done according to industry-standard IPM protocols.
QUALITY CONTROL PLAN OVERVIEW
Our Quality Control Program will work to ensure that the contract requirements are met or exceeded by Pestmaster
Services. The service technician‟s phone number as well as the company‟s contact information is available to the COR for notification of any concerns. The company‟s contact information is available for any concerns regarding the technician. All personnel concerns will be reviewed by a supervisor at least one management level above the person involved and appropriate action including warning(s) and/or termination will be taken. An employee‟s undesirable performance, such as not practicing IPM methods or poor service, will not be tolerated. Depending on the severity of the infraction, immediate termination may be enacted.
Either the lead Customer Service Representative or the Quality Control Representative will randomly call facility managers to perform quality control surveys. They will use a “QC Form” that request answers to a number of questions.
These will include questions on:
Neatness of Technicians
Timeliness
Procedures used (IPM)
Request Suggestions to Improve our Services.
Service logs will be filled out and the success of the procedures used will be evaluated. If a review of the service logs or
QC calls document performance concerns or poor pest control results, we will immediately dispatch technicians to correct deficiencies, at no extra charge to the government. Service logs are available for review by the client.
