About this file
This document is a Past Performance Questionnaire template used for source selection evaluation in the NextSTEP-3 federal contract procurement. The questionnaire is sent to organizations with first-hand experience working with potential contractors to gather performance data as part of NASA's evaluation process. Responses must be submitted electronically to Jennifer George, the Procuring Contracting Officer at MSFC, no later than June 15, 2026, to support the source evaluation and selection process.
The questionnaire contains five sections requesting comprehensive information about contractor performance. Section 1 captures contract identification details including contractor name, CAGE code, contract number, type, program title, period of performance, dollar value, and work description. Section 2 identifies the customer/agency and geographic scope of services. Section 3 collects respondent information including name, title, contact details, position, and tenure on the contract. Section 4 contains structured performance ratings using a standardized six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) across three evaluation categories: Technical Performance (six questions addressing scope understanding, deliverable quality, technical standards, system engineering, quality assurance, and professionalism); Program Management (twelve questions covering contract management, personnel qualification, problem resolution, responsiveness, schedule flexibility, subcontractor management, communication, cost alternatives, process improvements, and workforce hiring/retention); and Cost Performance (four questions on cost forecasting, performance within budget, cost alerts, and reporting). Section 5 requests overall comments regarding the contractor's ability to execute proposed work and additional remarks. All completed questionnaires become part of official source selection records and are treated as Source Selection Information under FAR 2.101 and 3.104.

NextSTEP-3 Appendix E Attachment E: Past Performance Questionnaire
For Official Use Only Page 1 of 7
One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. To meet the acquisition milestones, we request your written response no later than June 15, 2026. This schedule will allow us sufficient time to analyze the data as an integral part of the source evaluation and selection process.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to Jennifer George at msfc-nexus@mail.nasa.gov. Your complete questionnaire will become a part of the official Source Selection records.
Your assistance is greatly appreciated, and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
Signature, Jennifer George, MSFC/PS50 Procuring Contracting Officer
