About this file
This is a Prime Proposal Checklist for CMS solicitation 75FCMC26R0041 designed to help offerors organize and present key proposal information in a standardized format.
The checklist requires prime contractors to submit company identification details (name, address, point of contact, telephone number), parent company information if newly formed, proposal validity period, and a list of proposed subcontractors. Contractors must provide their Tax Identification Number (TIN), Unique Entity Identifier (UEI), and CAGE Code, along with productive hours data for exempt and non-exempt employees. The checklist also mandates that offerors identify their business size under NAICS Code 541990, with options to designate as small business with additional classifications including disadvantaged, woman-owned, veteran-owned, HUBZone, or 8(a) status.
A critical component of the checklist addresses compliance with 13 C.F.R. 125.6 regarding limitations on subcontracting and small business designations. Offerors must self-verify and declare one of three scenarios: no subcontractors with 100% prime performance, prime receiving at least 50% of personnel costs with subcontractors, or prime and similarly situated subcontractors collectively receiving at least 50% of personnel costs. For each subcontractor, contractors must document business name, size designation, UEI number, work description, similarly situated status, and any further subcontracting activity. The checklist includes detailed penalty warnings for violations of subcontracting rules, with potential fines up to $500,000 or the amount spent in excess of permitted levels, and notes that contracting officers may require compliance demonstration during or after contract performance through invoices, subcontracts, or task value documentation.
