Instructions for Tab 1 & Tab 2
| Instructions for the Staffing Matrix (Tab 1); include the following: | |
| Labor Category/Position Title | |
| Position # (offeror numbering preference is acceptable) | |
| Cross-utilization - indicate position being cross-utilized with an asterisk (*) and identify the other position number being cross-utilized with | |
| PWS paragraphs covered by position(s) proposed | |
| If the prime or subcontractor is proposed to perform for each labor category/position | |
| Associated hours necessary to accomplish PWS requirements proposed for each labor category/position | |
| Number of full-time equivalents (FTEs) based on hours proposed for each labor category/position | |
| Provide a separate staffing matrix for each technical subfactor. | |
| For Technical Subfactor 1&3, provide a staffing matrix for a 12 month base period, plus, four one-year option periods. | |
| For Technical Subfactor 2&4, provide a staffing matrix for a 12 month base period. | |
| All site-applicable PWS paragraphs must be accounted for within the matrices, down to the subparagraph-level | |
| NOTES: | |
| 1 | Offerors' format is acceptable as long as it includes all the required elements within the templates and examples; Microsoft Word or Excel may be used |
| 2 | DO NOT include PWS Paragraph ranges (eg., X.1. - X.2 or X.1 through X.2) as this complicates Proposal Evaluation |
| 3 | Ensure Labor Categories/Position Titles and #'s match the required manning as applicable. |
| 4 | Ensure the period of performance indicated under each technical subfactor is covered. |

| Relevant Experience: |
| 4-8 years of experience as a Facility Manager. Familiar with advanced civil engineering practices, HVAC, power, plumbing, fuel, and HAZMAT operations and has 2+ years of supervisory experience. Familiar with autoCAD and able to operate normal office equipment to include computers, faxes, and copiers. Experience in construction work and operation and maintenance of electric utilities, including facility backup generators and uninterruptible power supplies. |

| Associates Degree in Engineering/Mechanical/Technical field, or equivalent experience. |

| - Electrician Training and certification by applicable State/National Code | |
