About this file
This is a Worker Protection and Investment Certification Form (BOP 2201) issued by the Commonwealth of Pennsylvania in conjunction with a Tree Trimming Stump Removal Services procurement for PennDOT in Washington County. The certification form requires all contractors and grantees to verify compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and applicable state labor and workforce safety laws, including but not limited to the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and multiple other statutory requirements. A duly authorized official must execute the certification under penalty of perjury, confirming the contractor's or grantee's compliance status and acknowledging the obligation to immediately report any changes in compliance to the Purchasing Agency.
The certification carries significant penalties for false statements, including contract termination and a three-year ineligibility to bid on Commonwealth contracts under 62 Pa. C.S. 531 (Debarment or Suspension), as well as criminal penalties under 18 Pa. C.S. 4904 (Unsworn Falsification to Authorities). This certification requirement applies to all bidders for the Tree Trimming Stump Removal Services contract; notably, this procurement is restricted to DGS-certified small businesses under Pennsylvania's Small Business Procurement Initiative (Executive Order 2011-09), and all bids must be submitted electronically through the PA Supplier Portal.
