Operation and Maintenance Items Date : 04/22/2008
Page : 1Center : DENISON (00702) of 3
One-Liner
Emergency generator not periodically tested under load
Combustion air not provided
Make-up air not provided
Piping installed without a cleanout to sanitary drain
Ventilation is not provided in medical supply room
Item
Center maintenance staff have not been periodically running the emergency generator under loaded conditions.
(NFPA 101, 2003, 9.1.3 and NFPA 110, 2002, 8)
The building has a mechanical room with a gas-fired appliances. The mechanical room has no provision for combustion air other than the louvered doors.
(IMC, 2003, 703.1.)
The kitchen hood fans exhaust a high volume of air, but lack provisions for being interlocked with the make up air fan. This is causing a significant negative pressure in the kitchen.
(IMC, 2003, 508.1)
Sanitary drain is not provided with a cleanout at the base of a stack.
(IPC, 2003, 708.3)
The medical supply room is not ventilated.
(IMC, 2003, 403.1)

Provide e-mail notification to Building 1 and otherwise coordinate as necessary for facilitating a 4 hour outage to the normal power system of the building, one time each month. Shut down main power to the building and restore after four hours of run time on the generator, under loaded conditions. Thoroughly inspect transfer switches, generator operation, and note specific abnormal effects to the building which can later be corrected. Update generator maintenance log with test results, run time duration and date. This is additionally the ideal time to provide code required monthly inspection of the building's life safety lighting and exit signage, and also the associated center's safety and security documentation in accordance with NFPA 101, 2003, 7.9.3 for this building.
Remove blank-offs from the existing louvers in the doors. Provide mechanical room with two openings for combustion air, one within 1 foot of the ceiling and one within 1 foot of the floor. The two louvers should be 44" x 22" in size and selected with a net free area factor of 50%.
Provide interlock wiring between the exhaust fans and the make up air fan.
Install a cleanout fitting in the ceiling space above the basement level men's restroom.
Extend to, and terminate flush with, the floor or wall in the first floor level men's restroom.
Provide with a plug and a cover plate.
Install a supply, return or exhaust grille in the ceiling connected to an existing duct system. As an alternate method, provide two 10" x 8" sidewall grilles (one high, one low) in the wall of the room.

One-Liner
Ventilation not provided in masonry saw room
Fire extinguishers not provided
Roof ladder is unsafe
Outdoor air blocked
Thermostat missing cover
Inoperable humidifiers
Item
The masonry saw room located at the east exterior wall does not have a means of ventilation.
(IMC, 2003, 502.1)
Two of the three storage spaces at the east of the building do not have portable fire extinguishers.
(NFPA 10, 2002, 5)
Hand bars of the fixed metal ladder, leading from the south dormitory roof to the top level, are attached to the membrane roofing only. The ladder leader is loose at the top.
(IBC 2003, 115.1)
The outdoor air duct to a HVAC unit is significantly blocked (85%) by styrofoam. This located on the third floor, in the lounge. The blockage is a concern, but the material in the airstream is of serious concern.
A thermostat is missing a cover on the third floor, west wing.
Three humidifiers on the third floor are inoperable due to lack of maintenance. The cylinders that collect calcium in the supply water must be serviced and replaced. These humidifiers maintain a minimum space humidity level. This is required because of the four large outdoor makeup air units serving the building.

Provide a 100 CFM ceiling sidewall supply fan mounted in the interior wall. Extend relief 12" x 12" duct 4 LF from from screened opening and with integral backdraft damper through metal siding exterior wall and provide appropriate wall cap. Energize from lighting circuit, using the existing switch and 10 LF of 3 #12 in 12" EMT. Repair drywall if necessary.
Install, inspect, and maintain fire extinguishers in these areas of the building.
Bolt hand bars to the roof structure below and, if necessary, provide additional wall brackets to provide safe structure. Seal any necessary roof penetrations.
Remove the blockage.
Replace the cover.
Service the units including replacing the calcium collecting cylinders.

One-Liner
Exterior light fixtures operating during daylight hours
Water heater relief valves discharge to floor
Site lighting fixtures not operational
Fire alarm system inspection paperwork not adequately maintained
Item
The photocell for two building-mounted exterior lighting fixtures has failed. These fixtures are located at the back of the building. The condition causes the affected lighting units to remain on continuously. The condition not only shortens the life of the lamps and ballasts, but also wastes energy.
The two water heater relief valves discharge directly to the floor in the mechanical room.
(IPC, 2003, 504.6.1)
The pole-mounted site lighting fixture located just north of the flagpole at Building 1, and the HID spotlight mounted on Building 1 for illuminating the flags, are not operational due to burned out lamps, ballasts or an open circuit. Additionally, a building mounted HID fixture at the north side of Building 1, and another at the northeast end of Building 13 facing the street are not operational.
Although all fire alarm systems were fully operational on center as of the November 2005 Facility Survey, a number of code-required annual fire alarm inspection reports were not found. The inspection reports found were being retained in the panels.
