There is no paved sidewalk from the center to the athletic field.

The sudents/staff have to walk down the main road, Opportunity Road, and across the grass to reach the athletic field.

The athletic track surface is composed of large gravel which has been overgrown with weeds and grass and is not suited for running. Consequently, the track is not utilized. At the time of 2005 Facility Survey, the center presented a new program to convert this athletic area into soccer field.

Construct a concrete sidewalk/stair through the tree-lined hill side near Building No. 10 to a termination point of convenience near the athletic field. Provide pole mounted light fixtures along the walk path (2-10'-0" light fixtures at each end of the path and 10-4'-0" bollard light fixtures between) and provide the required electrical wiring and connections.

(approximately 600 LF)

Remove large gravel from the track field. Re-grade the whole field to establish a crown running the length of the center and sloping off at 1% to both sidelines, to provide natural drainage.

Provide four to six inches of topsoil over the rough grade. Seed a high quality grass. Apply a fertilizer uniformly over the field.

Approximate size of the soccer field should be 100 by 50 yards.

As of the November 2005 Facility Survey, this deficiency had not been corrected.

As of the November 2005 Facility Survey, this deficiency was still valid.

The pole-mounted site lighting fixture located just north of the flagpole at Building 1, and the HID spotlight mounted on Building 1 for illuminating the flags, are not operational due to burned out lamps, ballasts or an open circuit. Additionally, a building mounted HID fixture at the north side of Building 1, and another at the northeast end of Building 13 facing the street are not operational.

Although all fire alarm systems were fully operational on center as of the November 2005 Facility Survey, a number of code-required annual fire alarm inspection reports were not found.

The inspection reports found were being retained in the panels.

(NFPA 101, 2002, 9.6.1.10)

Contact the utility company to repair the light on the pole.

Replace lamps, ballasts and/or perform other required maintenance to return the building-mounted fixtures to normal operation.

Retain all code required maintenance and periodic inspection reports for the fire alarm systems. Keep the documents organized for easy access.

There are numerous complaints of discomfort by building occupants related to uneven temperature throughout the building. Each floor is divided into 5 to 7 zones, but temperatures vary widely within the zones, especially the large core zone. Each zone is provided with a single temperature sensor, many of which are not in a representative location. Air handler fans are single-speed and are loud. Duct air velocity causes some supply air diffusers to generate noise and excessive air movement degrades occupant comfort.

Replace the existing supply air diffusers with sixty 2' by 2' full face, lay-in, 12 inch round neck ceiling diffusers. Provide diffusers with integral dampers capable of being adjusted without removing the inner cone. Relocate zone temperature sensors to return air ducts. Retain the services of a certified HVAC system balancing contractor to calibrate the pneumatic controls and balance the air distribution system.

As of the November 2005 Facility Survey, this deficiency was still valid.

The storefront doors, located at the entrance vestibule are damaged due to the poor quality of aluminum frames and doors. The original material is inadequate to withstand the rugged use and weather. Doors and frames are damaged.

Cold air blows through the gaps of the aluminum frames, which causes significant temperature changes in the corridor and nearby offices.

Remove the exterior vestibule aluminum storefront. Replace with high quality steel/glass storefront doors, which will withstand heavy use in this high-traffic area. Provide two sets of 6' x 7' steel/glass doors with astragal in between door leafs.

Door stile should be 4-1/2" width, top rail 4-1/2" height, and bottom rail 10" height. Framing should be reinforced with steel and used with a continuous hinge for maximum strength and dependability. Total storefront is 23'-6" wide and 9' height.

Center maintenance staff have not been periodically running the emergency generator under loaded conditions.

(NFPA 101, 2003, 9.1.3 and NFPA 110, 2002, 8)

The building has a mechanical room with a gas-fired appliances. The mechanical room has no provision for combustion air other than the louvered doors.

(IMC, 2003, 703.1.)

Provide e-mail notification to Building 1 and otherwise coordinate as necessary for facilitating a 4 hour outage to the normal power system of the building, one time each month.

Shut down main power to the building and restore after four hours of run time on the generator, under loaded conditions.

Thoroughly inspect transfer switches, generator operation, and note specific abnormal effects to the building which can later be corrected. Update generator maintenance log with test results, run time duration and date. This is additionally the ideal time to provide code required monthly inspection of the building's life safety lighting and exit signage, and also the associated center's safety and security documentation in accordance with NFPA 101, 2003, 7.9.3 for this building.

Remove blank-offs from the existing louvers in the doors.

Provide mechanical room with two openings for combustion air, one within 1 foot of the ceiling and one within 1 foot of the floor.

The two louvers should be 44" x 22" in size and selected with a net free area factor of 50%.

The underground horizontal building sanitary sewer piping has been broken and has tree roots growing inside it in several locations. This creates frequent blockages and backs sewer up into the floor drains at the basement level (dining hall).

The medical supply room is not ventilated.

(IMC, 2003, 403.1)

Determine the locations of the blockages and excavate such that the broken piping can be repaired and replaced. Remove the tree roots in the area. For the purpose of estimating, assume excavating five feet deep, removing a four foot section of 6" cast iron pipe, replacing with same including sealing and the use of two mechanical joint couplings. Upon completion, backfill and tamp to the proper compression. This is to be done at four locations.

Install a supply, return or exhaust grille in the ceiling connected to an existing duct system. As an alternate method, provide two 10" x 8" sidewall grilles (one high, one low) in the wall of the room.

Center personnel noticed cracks in the floors and walls. With visual monitoring, they noted the increase in the cracking over the next few months. The most significant cracks were reported in the corridor, in the storage room and in the gym floor adjacent to the west wall.

This monitoring should include:

1. Vertical and horizontal monitoring of the west wall.

2. Vertical check of the floor level in the multi-purpose room

3. Vertical and horizontal monitoring of the top of the concrete retaining wall.

4. Groundwater monitoring. Two observation wells will be drilled.

5. Four ½ "diameter weep holes will be drilled in the retaining wall.

6. General regrading is needed around the building.

These points should be monitored every two weeks for the first month, followed by monthly monitoring for the next three months.

The cracks and joint gaps that have occurred thus far should be cosmetically repaired.

If more than an additional ½" of movement is reported, additional structural assessment/inspection should be performed (especially checking joist bearing lengths where there is movement). Further movements could cause more severe structural damage.
