About this file

This document is an Addendum 4 for a Request for Proposal (RFP DOH24-010) issued by the Florida Department of Health, specifically for the Division of Emergency Preparedness and Community Support (DEPCS) IT Hosting Services. The RFP seeks managed hosting services for a one-year initial term beginning December 01, 2025, with potential renewal options. Key modifications include changing the contract duration from five years to one year and replacing the original Attachment B Price Page with a revised version. The addendum includes a comprehensive set of questions and answers that clarify technical requirements, such as the necessity of a Tier 4 datacenter, server configuration, data storage needs, and notification processes for equipment and software upgrades.

The cost proposal is allotted 100 points and represents approximately 30 percent of the overall RFP score, with the lowest overall grand total receiving maximum points. Vendors are required to provide quarterly and annual pricing for managed hosting services as specified in the scope of services. Technical requirements include provisions for dedicated servers (both cluster and standalone), a Network Attached Storage (NAS) for data backup, and an estimated 20 monthly smart hands hours. The RFP does not specify preferred cloud/SAAS partners or specific bandwidth requirements, leaving those determinations to the respondent. The state is currently utilizing a managed hosting party for setup, administration, management, and support, and vendors must notify the Contract Manager within 14 business days of any equipment or software updates.
