About this file

This document is a comprehensive bid and contract package for Mt. San Antonio Community College District located in Walnut, California, pertaining to the Building 4 Mail Services Remodel project. The project involves the remodeling of approximately 1,200 square feet of existing space and includes demolition of walls, flooring, mailboxes, ceiling panels, and lighting, followed by installation of new walls, doors, windows, flooring, ceiling panels, lighting, electrical systems, and data connections. This is a prequalification phase only; contractors must successfully complete prequalification requirements and receive a Prequalification Approval Letter before being eligible to submit bids in the subsequent formal bidding phase. The package specifies that only firms receiving prequalification approval will be invited to participate in the competitive bidding process, and any previous prequalifications for other District projects do not apply to this solicitation.

The bid package requires submission of multiple forms and certifications including a Non-Collusion Declaration, Bid Bond (10% of bid amount), Performance Bond (100% of contract price), Payment Bond (100% of contract price), Workers' Compensation Insurance Certificate, Drug-Free Workplace Certification, Iran Contracting Act Certification, Department of Industrial Relations registration verification, Disabled Veteran Business Enterprise participation documentation, Form W-9, and vendor information forms. For non-California vendors, additional withholding forms (Form 587, 588, or 590) may be required. The District has established a three percent (3%) participation goal for Disabled Veteran Business Enterprises. Contractors must maintain current DIR registration throughout contract performance, and all subcontractors must be DIR-registered contractors. The prequalification application must include complete financial documentation, insurance requirements, and safety program information, with the District providing specifications, general conditions, and construction agreements for informational purposes during the prequalification phase.
