About this file

This is a Disqualification Disclosure form required by the City of Pasadena Housing Department in accordance with California Public Contract Code Section 10162 for the Federal Labor Compliance Officer consulting services solicitation. The form requires prospective bidders to certify under penalty of perjury whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate yes or no to this inquiry, and any affirmative responses require detailed explanation of circumstances with supporting documentation attached as an exhibit. This disclosure is a mandatory certification requirement for proposal submission, with questions due by May 4, 2026, at 12:00 p.m. and proposals due electronically through the City's eProcurement Portal by May 8, 2026, at 3:00 p.m. The selected consultant will be engaged for an initial one-year term with three optional one-year renewal periods subject to City Manager approval.

The Disqualification Disclosure form does not contain pricing information, set-asides, or funding details, as it functions solely as a mandatory certification and compliance document. All responding vendors must complete this form as part of their proposal package and comply with the City of Pasadena's broader procurement requirements, including active SAM registration with a valid Unique Entity Identifier (UEI). The form serves as a screening mechanism to ensure bidder eligibility and integrity throughout the competitive selection process for federal labor compliance services on HUD-funded construction projects.
