About this file

This is a Disqualification Disclosure form required by the City of Pasadena Housing Department in accordance with California Public Contract Code Section 10162 for the Federal Labor Compliance Officer consultant procurement. The form requires prospective bidders to certify under penalty of perjury whether the bidder, any officer of the bidder, or any employee with a proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation. Bidders must indicate whether their answer is yes or no, and if affirmative, must provide a detailed explanation of the circumstances along with supporting documentation.

This disclosure form is a mandatory compliance requirement for all bidders seeking to provide federal labor compliance services for HUD-funded construction projects, including those financed through the Community Development Block Grant and Home Investment Partnership programs. The form serves as a prerequisite qualification check to ensure that only contractors without disqualification histories are eligible to participate in the procurement process.
