About this file

The document is a Disqualification Disclosure form from California, specifically aligned with Public Contract Code §10162, intended for prospective bidders participating in government contract opportunities. The form requires bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.

The form provides a simple yes/no checkbox for bidders to indicate their past contract performance history, with an option to attach additional explanatory documentation if the answer is "yes". This disclosure serves as a mechanism for government entities to assess potential contractors' compliance and reliability before awarding contracts, ensuring transparency and accountability in the public procurement process.
