About this file
This document is a Facilities Maintenance Questionnaire for an 8(a) small business set-aside contract under NAICS code 561210. The questionnaire seeks to evaluate potential contractors' experience in building operations and maintenance for multi-story facilities. Key requirements include: demonstrating experience with maintenance work in office/administrative spaces, research laboratories, and data facilities; ability to dispatch personnel using a work order system; and self-performing at least 20% of the contract cost with company employees.
The document requires prospective contractors to provide detailed information about their past projects, specifically those involving multi-story facilities of at least 150,000 square feet. Contractors must submit project details such as current percentage of construction completion, project scope, size, and the portion self-performed. Additionally, contractors are asked to provide references from government agencies for recent (3 years or less) and relevant contracts, and are invited to offer any additional feedback regarding the Performance Work Statement (PWS).
