About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on July 20, 2017, designed to inform potential bidders of common bidding process requirements and disqualification criteria for state construction projects. The document serves as an informational guide rather than a comprehensive list of requirements for any specific project and emphasizes that all bidders must review the complete request for quotation (RFQ) and associated bid documents in their entirety to understand actual bidding requirements. The form identifies eleven errors that result in immediate bid disqualification, including failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to provide a West Virginia contractor's license number, failure to submit a signed drug-free workplace affidavit, failure to provide a valid bid bond, failure to meet mandatory RFQ requirements, failure to acknowledge receipt of addenda when required, late bid submission, federal debarment status, State of West Virginia debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints.
The document additionally lists three categories of errors that may result in bid disqualification before contract award, contingent upon resolution prior to award: uncontested debt to the State exceeding $1,000, delinquency in workers' compensation or unemployment compensation payments, and failure to obtain required bonds and/or insurance. The form explicitly states it is not intended to serve as an exhaustive checklist and that omissions or oversights not listed may still result in bid disqualification, placing responsibility on bidders to thoroughly review all bid documents and comply with stated requirements.
