About this file

This document is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) to provide guidance on bidding requirements and common disqualification errors for construction projects.

The form outlines mandatory requirements that bidders must meet to remain eligible for contract award. Immediate bid disqualification results from failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply a valid bid bond, non-compliance with mandatory RFQ requirements, failure to acknowledge receipt of addenda when stipulated as mandatory, late bid submission, federal or state debarment or suspension status, outstanding environmental enforcement actions, or unresolved DEP vendor complaints. Additional disqualifying factors that must be resolved prior to contract award include outstanding debt to the state or political subdivision, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to provide a West Virginia contractor's license number within one day of request, failure to submit a signed drug-free workplace affidavit within one day of request, and failure to provide a subcontractor listing within one business day of bid opening or DEP request.

The form serves as an informational reference and explicitly states it does not constitute a comprehensive list of all bidding requirements or potential disqualifying errors. All prospective bidders are required to thoroughly review all bid documents, including the request for quotation, addenda, and related instructions, to ensure full compliance with stated requirements. The form emphasizes that oversight or failure to comply with stated requirements may result in bid disqualification regardless of whether such errors appear on this reference list.
