About this file

DD Form 3058 is a Department of Defense Consent to Conduct Installation Records Check (IRC) form prescribed by DoDI 1402.05, designed to collect criminal history information on DoD personnel and contractors seeking to work with children in DoD child care services programs.

The form serves as an authorization and release document that enables the DoD to conduct background checks across military law enforcement records, the Defense Central Index of Investigations (DCII), and Family Advocacy Program (FAP) records relating to child and domestic abuse, including state-specific child abuse and neglect registries. Subject information collected includes name, date of birth, place of birth, Social Security number, and current address. The form requires voluntary consent from the subject or parent/legal guardian, with the understanding that failure to provide requested information may result in an unfavorable determination regarding suitability to work with children. Section III requires identification of the employing command/installation, position hire/start date, and position category, which includes options for civilian employees (APF/NAF), contractors, military personnel, volunteers, in-home care providers, family members, teen employees, JROTC instructors, and other classifications. Section IV documents the completion of three types of background checks—Family Advocacy Program, Installation Law Enforcement, and DCII (optional)—specifying whether each check was initial, annual, or five-year, with certifying officials required to confirm that no disqualifying information was found. The form is controlled by OUSD(P&R) and carries a privacy category classification of PRVCY with FEDCON distribution control.
