About this file

This document is an Attachment D.2 Reference Contract Worksheet for evaluating an offeror's experience providing HCHV (Homeless Committed Veterans) Transitional Housing and Supportive Services. The worksheet requires detailed information about previous contracts, including the legal name of the entity with whom the contract was held, contract number, confirmation of HCHV transitional housing services, specific tasks performed, geographic location of services, contract dates, number of patients served, prime or subcontractor status, and contact information for reference verification.

The document emphasizes that general statements about experience are not acceptable, and failure to provide specific, detailed contract references may be interpreted as evidence of lacking experience in HCHV transitional housing services. The worksheet is designed to help the government comprehensively evaluate proposals under the Experience evaluation factor, with a clear warning that incomplete or vague submissions could negatively impact the offeror's chances of contract award.
