About this file
This is a Quality Assurance Surveillance Plan (QASP) for fire alarm and fire sprinkler inspection services at the Sioux Falls VA Health Care System located at 2501 West 22nd Street, Sioux Falls, South Dakota 57105. The contractor is responsible for mobilization to the facility and proper documentation of fire sprinkler and fire alarm data and reports, furnishing all necessary supervision, labor, materials, and equipment. The contractor must comply with standard VA policies including dress codes, identification badges, and parking requirements.
The QASP establishes performance standards monitored through two primary tasks: scheduling inspections annually, semiannually, and quarterly per SOW intervals (aligned with NFPA allowances), and executing inspections with complete reports submitted to the VA timely. For scheduling inspections, exceptional performance requires completion 15 days or earlier before the due date, satisfactory performance requires completion within 5 days before the due date, and unsatisfactory performance occurs beyond 20 days from the due date. For executing inspections and submitting reports, exceptional performance requires reports within 10 calendar days or less, satisfactory performance requires reports within 14 calendar days, and unsatisfactory performance occurs beyond 14 calendar days. The Contracting Officer's Representative (Aaron Kompelien, primary; Adam Thompson, alternate) conducts routine monitoring through QA reports, contractor performance evaluation reports, and CPARS evaluations. The COR shall meet with the contractor at least annually to assess performance. Unacceptable performance triggers Contract Discrepancy Reports requiring written acknowledgment and corrective action plans as specified by the Contracting Officer.
