About this file
This is an internal VA guidance document that outlines which staff members are authorized to complete Suicide Prevention Safety Plans and Suicide Behavior and Overdose Reports (SBOR) within VA facilities.
The document provides a detailed table indicating which medical and mental health professionals can complete these tasks, with both requiring local facility Chief of Staff concurrence and being within facility scope of practice. Medical doctors, licensed psychologists, clinical pharmacist practitioners, licensed social workers, marriage/family therapists, mental health counselors, registered nurses, nurse practitioners, physician assistants, and rehabilitation counselors are authorized to complete both Safety Plans and SBORs. Licensed practical nurses, peer support specialists, unlicensed assistive personnel, psychological technicians, and social work assistants are not authorized to complete either document. Trainees in authorized categories may complete these tasks with appropriate co-signers.
